摘要
目的探讨血液透析（HD）联合血液灌流（HP）治疗尿毒症患者的临床疗效。方法选取新疆医科大学第一附属医院血液净化中心2010年3月-2012年12月尿毒症透析患者40例。将40例慢性尿毒症患者随机分为治疗组和对照组，每组各20例，对照组采用单纯血液透析治疗，每周3次，每次4h。治疗组血液透析联合血液灌流，治疗2h后，取下灌流器，继续HD治疗，两组均连续治疗2W。检测两组治疗前和治疗后血液尿素氮（BUN）、肌肝（Scr）、甲状旁腺激素（iPTH）、β2微球蛋白（β2-MG）含量并进行比较，观察两组皮肤瘙痒、食欲差、头晕、骨痛、高血压等症状改善情况。结果两组治疗后BUN、Scr水平均降低，但差异无统计学意义（P〉0．05）。治疗组治疗后iPTH、β2-MG较治疗前明显降低，差异有统计学意义（P〈0．05）；对照组治疗前、后iPTH、β2-MG无明履变化，差异无统计学意义（P〉0．05）；两组治疗后iPTH、β2-MG降低程度比较差异有统计学意义（P〈0．05），治疗组症状改善显著好于对照组（P〈0．05～0.01）。结论血液透析联合血液灌流治疗可有效清除血液血分子毒素，减轻临床症状，增加血液净化疗效，提高尿毒症患者的生活质量。
Objective To investigate the effect of hemoperfusion combined with hemodialysis on uremia. Methods Forty patients with chronic uremia were randomly divided into two groups. One for hemoperfu- sion combined with hemodialysis treatment, another for hemodialysis treatment only for 2 consecutive weeks. BUN, Scr, iPTH, the β2 MG level were assayed in blood samples before and after intervention. Itchy skin, poor appetite, dizziness, bone pain, high blood pressure symptoms were observed in two groups. Results After intervention, BUN, Scr, iPTH, the β2-MG level decreased more in combination group than that in the control group （P 〈0.05）, and the improvement of all symptoms was more than the control group （P 〈0.05）. Conclusion Hemoperfusion combined with hemodialysis to treat uremia can ef- fectively remove middle size toxin and attenuate symptoms and improve the life quality.
