摘要
目的探讨鼻眶筛（NOE）骨折导致的鼻泪管阻塞行鼻内镜下手术治疗的体会。方法回顾性分析第三军医大学大坪医院耳鼻咽喉头颈外科2008年6月-2012年6月收治的25例（27眼）NOE骨折导致的鼻泪管阻塞患者的临床资料及随访结果。对NOE骨折合并鼻泪管阻塞的患者行鼻内镜下泪囊鼻腔造口术，并同期对鼻中隔偏曲、鼻腔黏连等病变行手术治疗。结果术后随访6～12个月，本组25例（27眼）中，21例（23眼）治愈，3例（3眼）好转，1例（1眼）无效，手术治愈率85．2％（23／27眼），有效率96．3％（26／27眼）。结论应当重视NOE骨折导致的鼻泪管阻塞，经鼻内镜下泪囊鼻腔造口术治疗的效果满意。
Objective To discuss the surgical treatment of nasolacrimal duct obstructions secondary to naso-orbito-ethmoidal fractures. Methods The clinical data and following-up results were retrospectively reviewed from 25 patients （ 27 eyes ）, who were treated in the Department of Otolaryngology Head and Neck Surgery, Daping Hospital,The Third Military Medical University from Jun. 2008 to Jun. 2012. For patients who suffered from naso- lacrimal duct obstructions secondary to naso-orbito-ethmoidal fractures, nasal endoscopic dacryocystorhinostomy and other nasal endoscopic surgeries were performed. Results The postoperative follow-up duration was 6 to 12 months. A total of 21 cases（ 23 eyes） were cured successfully,3 cases were relieved and 1 case was failed. The success rate was 85.2% ,and the effective rate was 96. 3%. Conclusion Nasolacrimal duct obstructions should be attached more importance in the patients with naso-orbito-ethmoidal fractures. Endoscopic endonasal dacryocystorhi- nostomy is effective in treating nasolacrimal duct obstructions following naso-orbito-ethmoidal fractures.
