摘要
目的探讨早期肠内营养(EN)与肠外营养(TPN)对重症急性胰腺炎患者的治疗效果。方法将大庆油田总医院2010年1月至2011年12月收治的80例重症急性胰腺炎患者按入院时间顺序随机分成EN组和TPN组,EN组40例患者给予常规治疗,TPN组40例患者在给予常规治疗的同时,在患者胃肠道功能刚恢复后早期放置鼻空肠营养管并过渡到EN,通过比较两组治疗前后患者的营养状况、临床效果、住院时间、住院费用及并发症等指标,进行分析。结果 EN组在营养状况方面显著优于TPN组,并发症发生率显著低于TPN组;且血淀粉酶及尿淀粉酶的恢复时间、腹痛减轻时间、住院时间、住院费用及中转手术率和病死率均低于TPN组(P<0.05)。结论 EN支持治疗重症急性胰腺炎的疗效确切,不良反应少,可有效减少患者住院时间及医疗费,显著优于TPN支持治疗。
Objective To study the early elinieal therapeutie effect of enteral nutrition and parenteral nutrition in severe acute panereatitis patients. Methods Aeeording to admission time sequenee 80 patients of severe acute panereatitis patients treated in Daqing Oil Field General Hospital from January 2010 to Deemnber 2011 were randomly divided into two groups：control group of 40 patients who were given conventinnal treatment and observation group of 40 patients who were given early placed nasal jejunum nutrition tube and transition to enteral nutrition after recovering of gastrointestinal function ; nutrition status, clinical effect, hospital stays, expenses and complications before and after treatment of the two groups were eompared. Results Nutritional status of the observation group was significantly superior to the control group,the inci- dence of complications was less than that of the control groups ; and recovery time of blood amylase and urine amylase, the time of reducing abdominal pain, hospital stays, hospitalization expenses and transfer operation rate and mortality were lower than those of the eontrol group （ P 〈 0.05 ）. Conclusion The enteral nutrition support therapy of severe acute panereatitis has exact curative effect and less adverse reactions, which can effectively reduce hospital stays and expenses and is superior to parenteral nutrition significantly.
