江西丰城手足口病200例临床分析 被引量:1

Clinical Analysis of 200 Cases with Hand-foot-mouth Disease in Fengcheng Area

摘要 目的探讨江西省丰城市手足口病的发病情况,为手足口病的防、治、控提供理论依据。方法对2012年3月至5月丰城市人民医院收治的200例手足口病患儿的临床表现、诊断及治疗措施进行回顾性分析。结果手足口病多发生于3周岁以下儿童,占92%(184/200),大部分患儿临床表现较轻,预后良好。>60%的患儿有发热,约1/4的患儿有肢体抖动。实验室检查近1/2白细胞总数升高,少数患儿血糖异常,极少数心肌酶谱异常。病原学检查CoxA16核酸阳性者占13.2%,EV71核酸阳性者占51.0%,CoxA16及EV71核酸均阴性者占39.6%。重症患儿大多以惊跳等中枢神经系统症状为主,占25.5%(51/200)。结论目前江西丰城地区手足口病患儿大部分仍以轻型为主;感染的病原体主要为EV71,其次为CoxA16。在县一级医疗机构,对手足口病的诊断方式主要为临床诊断。基层医疗机构应加强对医务人员的全员培训,加强重症手足口病救治工作,降低病死率。 Objective To explore the incidence of HFMD in Fengcheng area, and provide theoretical basis for the prevention, cure and control of HFMD. Methods Clinical data including clinical manifestation,diagnosis and treatment of HFMD admitted by People's Hospital of Fengcheng City during March and May 2012 were retrospectively analyzed. Results HFMD occurs more often in children under 3 years old, taking about 92% ( 184/200 ) , which was mostly mild and the prognosis was good. More than 60% of cases had fever, about a quarter of the cases had limb jitter. Nearly half of the cases had total leukocyte increase,a few cases had blood glucose abnormalities, very few cases had myocardial enzyme spectrum anomaly. Etiology check found that CoxA16 nucleic acid of 13.2% of the patients was positive while EVT1 nucleic acid of 51.0% of the patients was positive. CoxA16 and EVT1 nucleic acid were both negative in 39.6% of the patients. 25.5% (51/200) of the patients were severe cases which had the central nervous system symptoms such as startle reaction. Conclusion Most patients with HFMD in Fengcheng area were mild. The main pathogen was EV71 ,followed by CoxA16. The main diagnostic way of HFMD is clinical diagnosis in the medical institutions at the country level. Basic medical institutions should strengthen the medical personnel's overall training, strengthen the treatment of severe cases, and reduce the mortality.
