摘要
目的 通过分组比较盲肠结扎穿孔(CLP)与腹腔持续置管引流(CASP)脓毒症大鼠模型.方法 将126只Wistar大鼠随机分为4组,即正常组、假手术组、CLP组和CASP组;CASP组根据不同大小静脉留置针(规格型号:14G、16G和18G)分为3个亚组.分别于建模后的6、12、24、48 h记录大鼠体质量变化和采血,应用酶联免疫吸附试验(ELISA)检测外周血白细胞介素(IL)与、-10及肿瘤坏死因子(TNF)-α水平,并观察大鼠术后72 h生存率.结果 CLP组的72 h生存率为45.8%.在CASP模型中,18G组生存率为50.0%,高于16G组的33.3%和14G组的8.3%(P<0.01).CASP-14G、16G和18G组24 h时的体质量差值分别为(17.1±7.3)、(14 6±4.4)和(14.2±5.0)g,均高于CLP组(8.5±3.5) g(P <0.05).CASP组在各时间点的IL-6、IL-10及TNF-α水平明显高于CLP组(P<0.01).随着升结肠置管管径的增大,IL-6、IL-10及TNF-α水平也随之升高(P<0.05,除48 h时IL-10及TNF-α外).术后24 h尸体解剖发现,CLP组大鼠腹腔内感染灶大多形成局部炎性包裹,而CASP组大鼠腹腔脏器水肿充血明显,肠淤积,但未见明显炎性包裹形成.结论 本研究建立并对比两种常用脓毒症大鼠模型;CLP模型侧重于局限性腹腔脓肿形成的病理变化,而CASP模型偏向于弥漫性腹膜炎的临床病程.
Objective To compare cecal ligation and puncture (CLP) vs. colon ascendens stent peritonitis (CASP) sepsis rat models. Methods A total of 126 Wistar rats were randomly divided into four groups: normal control group, sham group, CLP group and CASP group. The CASP group was subdivided into 3 subgroups according to the diameter of the inserted stent ( 14-, 16-, and 18-gauge). The 72 h sur- vival rate was observed after operation. The body weight change, and serum interleukin (IL) -6, IL-10 and tumor necrosis factor (TNF)-α levels were analyzed at 6, 12, 24 and 48 h after operation. Results The 72 h survival rate in CLP group was 45.8% and 50% in CASP-18G subgroup, which was higher than in CASP-16G subgroup (33.3%) and CASP-14G subgroup (8.3%). There was significant difference among the three CASP subgroups ( P 〈 0. 01 ). Body weight change in CASP group at 24 h was higher than in CLP group (P 〈 0.05). The serum levels of IL-6, IL-10 and TNF-α in CASP group at each time point were sig- nificantly higher than in CLP group (P 〈 0.01 ). With the diameter increasing of the inserted stent, serum IL-6, IL-10 and TNF-α levels were elevated (P 〈0. 05, except for IL-10 and TNF-α at 48 h). Abdominal situs was analyzed after 24 h and we found an intra-abdominal abscess exhibiting in CLP group, but CASP rats had massive edema, paralysis of the intestine and vasodilation of intestinal vessels. Conclusion This study has established and compared two rat models of sepsis. The CLP model focused on the pathological changes of intra-abdominal abscess formation, and CASP model was similar to the clinical course of diffuse peritonitis.
