自动乳腺全容积成像冠状面图像在乳腺肿瘤鉴别诊断中的应用价值 被引量:14

摘要 目的探讨自动乳腺全容积成像技术(ABVS)冠状面图像在乳腺肿瘤鉴别诊断中的应用价值。方法回顾性分析90例患者(共92个乳腺肿块),其ABVS冠状面表现为边缘不光整,根据冠状面图像的特征分为边缘模糊不清、毛刺、成角、分叶4种,与手术病理检查结果对比分析。结果边缘毛刺征82个,其中74个呈"火山口"征,"火山口"征对浸润性导管癌的诊断符合率达100%。其余类型乳腺恶性肿瘤ABVS冠状面表现呈多样性,有成角、分叶、模糊不清。其中11个肿块经病理检查证实为乳腺良性肿瘤。结论 ABVS的冠状面"火山口"征对于乳腺恶性肿瘤有较高的敏感性,但也有部分良性肿块冠状面呈不光整边缘,需结合其他检测手段客观进行鉴别诊断。 Objective To assess the value of the automated breast volume scanner (ABVS) in differential diagnosis of breast neoplasms. Methods The clinical data of 90 patients with 92 breast lumps, who underwent ABVS examination, were ret- rospectively analyzed. The appearances on coronal plane of ABVS were classified into 4 types: indistinct margin, splculated, angular and microlobulated. The ultrasound manifestations were compared with histopathological findings. Results Among 90 patients 82 presented splculated type, including 74 cases appeared retraction phenomenon, all of whom were diagnosed as in- vasive ductal carcinoma; while other type of breast malignancy presented diversity in ABVS coronal plane as angular, microlob- ulated or indistinct margin. Other 11 cases were pathologically diagnosed as breast benign tumor. Conclusion The ABVS coro- nal plane retraction phenomenon has a high sensitivity for diagnosis of breast cancer, but some benign masses also present margin circumscribed, in this case other diagnostic approaches are needed.
