摘要
目的探讨早期肢体功能锻炼对脑卒中偏瘫并发抑郁症患者抑郁及肢体康复的影响。方法将100例脑卒中偏瘫并发抑郁症患者按治疗方法的不同分为2组:对照组和试验组,每组50例。2组患者均采用常规治疗,并进行健康宣教、心理咨询及心理疏导等常规护理。在此基础上,试验组患者于生命体征稳定48h后,根据肢体功能障碍的不同性质、部位进行早期肢体功能锻炼。观察2组患者治疗前、治疗8周后运动功能、日常生活活动能力、Zung抑郁自评量表(SDS)及汉密顿抑郁量表(HAMD)评分的情况。结果 2组患者治疗8周后上田敏偏瘫上肢、下肢和MBI评分均较治疗前明显升高,而SDS、HAMD评分均较治疗前明显下降(均P<0.05);试验组患者治疗8周后上田敏偏瘫上肢、下肢和MBI评分均较对照组升高更明显,而SDS、HAMD评分均较对照组降低更明显(均P<0.01)。结论脑卒中并发抑郁症患者尽早行肢体功能锻炼,能改善患者的抑郁症状,并有利于患者肢体功能恢复,提高运动能力和日常生活活动能力。
Objective To investigate the influence of early limb function exercise on depression and physical rehabilitation in patients with stroke hemiplegia complicated by depression.Methods One hundred patients with stroke hemiplegia and depression were randomly divided into control group and experimental group,with 50patients in each group.All patients were given conventional therapy and health education,psychological counseling,psychological consulting and other routine nursing activities.After stable vital signs were maintained for 48hours,early limb function exercise was performed in the experimental group according to the nature and localization of the limb dysfunction.Motor function,ability to perform activities of daily life,self-reported Zung’s depression scale（SDS）scores and Hamilton depression scale（HAMD）scores were observed in both groups before and after treatment for 8weeks.Results After treatment for 8weeks,Ueda Satoshi standardized hemiplegic function and MBI scores increased but SDS and HAMD scores decreased in both groups（P＆lt;0.05）.Furthermore,the increase in Ueda Satoshi standardized hemiplegic function and MBI scores and the decrease in SDS and HAMD scores in experimental group were more obvious than those in control group（P＆lt;0.01）.Conclusion Early limb function exercise should be performed to improve depressive symptoms,promote the recovery of limb function and enhance the ability to perform sporting and daily life activities in patients with stroke hemiplegia complicated by depression.
