摘要
目的通过门诊处方点评，促进医师合理开具处方。方法对吉林大学第一医院二部（以下简称“我院”）2010、2011、2012、2013年第一季度共39个月的处方评价登记表进行统计分析。结果2010．2013年第一季度平均每张处方开药品种数为1．71种；抗菌药使用率为24．92％；注射剂使用率为43．65％；药品通用名称使用率为100．00％；基本药物使用率为37．03％；平均每张处方金额149．00元，处方合格率为86．14％。其中，注射剂使用率由2010年的52．36％降至36．19％，差异有统计学意义（P〈0．05）；国家基本药物使用率由2010年的30．08％升至44．94％，差异有高度统计学意义（P〈0．01）；处方合格率由74．01％升至98．77％，差异具有高度统计学意义（P〈0．01）。我院不合理处方主要集中在用法用量不适宜（42．53％）、处方诊断缺失（35．14％）等方面，并以内科为主。经过定期培训、纳入质量管理控制体系等一系列改进措施后，2013年第一季度处方合格率达到98．77％。结论我院医师处方基本合理，通过处方点评，有效地促进了我院药物的合理应用。
Objective To ensure physicians write prescriptions reasonably, through the outpatient prescription comment. Methods From 2010 to the first quarter of 2013 statistical data of the prescriptions of the First Hospital of Jilin University- Division Ⅱ （＂our hospital＂ for short） were analyzed by prescription comment form. Results From 2010 to the first quarter of 2013, the average number of drugs of each prescription was 1.71 species; the percentage of antibiotic drug usage was 24.92%; the percentage of injection usage was 43.65%; general name use percentage was 100.00%; the percentage of essential drug usage was 37.03%; the average prescription amount was 149.00 Yuan, and the passing rate of prescriptions was 86.14%. The percentage of injection usage reduced from 52.36% to 36.19%, with statistic difference （P 〈 0.05）. The percentage of essen- tial drug usage improved from 30.08% to 44.94%, with significant statistic difference （P 〈 0.01）. The passing rate of prescrip- tions improved from 74.01% to 98.77%, with significant statistic difference （P 〈 0.01）. The unreasonable prescriptions were mainly centered on dosage errors （42.53% of unreasonable prescriptions） and diagnosis deletion （35.14% of unreasonable prescriptions）, mainly in inteInal medicine department. Through regular training and quality management control systems, the passing rate of prescriptions improved to 98.77% in 2013. Conclusion The prescription is basically reasonable in our hospital, and prescription comment can effectively promote reasonable use of medicine in our hospital.
