摘要
目的探讨冠心病心力衰竭患者合并贫血情况及其对预后的影响。方法选择2009年1月—2011年1月我院心内科收治的冠心病慢性心力衰竭(CHF)患者121例,按NYHA心功能分级将其分为心功能Ⅱ级组、心功能Ⅲ级组、心功能Ⅳ级组及按血红蛋白(Hb)水平将其分为Hb正常组、轻度贫血组及中度以上贫血组。采用前瞻性队列研究的方法,在接受常规控制心力衰竭治疗情况下,分析冠心病CHF不同心功能分级患者合并贫血情况及其对预后的影响。结果随着心功能恶化,冠心病CHF患者贫血发生率逐渐升高,差异有统计学意义(P<0.05)。随访期间,贫血程度越重,冠心病CHF患者病死率、平均住院次数和平均住院时间越高,差异有统计学意义(P<0.05)。结论冠心病CHF患者贫血的发生率随着心功能的恶化而增加;合并贫血的患者其病死率、平均住院次数较高和平均住院时间较长,且贫血程度越重,其病情也越严重,预后越差。贫血是冠心病CHF患者预后独立的预测因子。
Objective To study the status of anemia in patients with coronary heart disease heart failure and its influ- ence on the prognosis. Methods 121 patients with coronary heart disease and chronic heart failure （CHF） were chosen from January 2011 to January 2013. They were divided into NYHA 11 group, NYHA lff group, NYHA IV group according to NYHA class, and they were divided into normal group, mild anemia group and moderate anemia, according to hemoglobin （Hb） lev- els. A prospective cohort study was used, in an interview with the case of the conventional control treatment of heart failure, pa- tients with coronary heart disease heart failure and its influence on the prognosis were analyzed. Results With the deterioration of heart function, incidence of anemia in patients with coronary heart disease and CHF gradually increased （P 〈 0. 05）. During follow- up, the more severe anemia, coronary heart disease mortality in patients with CHF, the average number of hospitaliza- tions were higher and average length of stay were longer （ P 〈 0. 05 ）. Conclusion The incidence of anemia in CHF with coro- nary heart diseases increasing with deterioration of cardiac function ; The mortality, the average number of hospitalizations and av- erage length of stay of patients with anemia are higher, and more severe anemia, the more serious the disease, the worse the prognosis. Anemia is independent predictors of coronary heart disease patients with CHF.
