摘要
目的了解临床静脉滴注中药注射剂医嘱情况,为中药注射剂的合理使用提供参考。方法对2013年5月1～31日使用注射用红花黄色素、注射用血栓通、肾康注射液和康艾注射液的出院患者随机抽取400份医嘱。通过医院HIS系统查阅电子病历及用药医嘱,对住院医嘱的适宜性进行点评。结果发现的主要问题为溶媒用量不适宜179份(44.75%)、适应证不适宜155份(38.75%)、重复用药99份(24.75%)、溶媒品种不适宜57份(14.25%)、给药疗程不适宜38份(9.5%)、药品用量不适宜35份(8.75%)。结论中药注射剂不适宜医嘱较多,药师应对医嘱进行审核并要求医师改正错误医嘱,保证用药安全有效。
Objective To analyze the medical records of Traditional Chinese Medicine（TCM）injections in our hospital in or- der to provide the reference for rational use of the injections. Methods Medical records of 400 discharged patients who were adminis-tered with intravenous infusions of safflower yellow injection, zhusheyong xueshuantong, shenkang injection or KA injection from May 1, 2013 to May 31,2013 were randomly sampled. The use of TCM injections were inspected by HIS system and their rationality were thus commented. Results The main problems found in records were as follows ： the insufficient amount of solvent was found in 179 orders （44.75%） ,incorrect indications in 155（38.75% ） ,repeated administration in 99 （24.75%）, incorrect solvent in 57 （14.25%）, in-correct dosing regimens in 38 （9. 5 % ）and unsuitable drug dosage in 35 （8. 75% ）. Conclusion There are many unsuitable medical re-cords for TCM injections. The pharmacist shouled be responsible for reviewing the orders and require the physicians to revise it, thus, ensure the safe and effective medication.
