摘要
目的：探讨应用腓骨肌腱鞘筋膜瓣与脂肪瓣联合转移修复外踝缺损的护理体会.方法：2008年1月～2011年8月用腓骨肌腱鞘筋膜瓣与脂肪瓣联合转移,修复外踝软组织缺损或钢板外露8例,根据患者不同年龄、不同病情,制定不同护理方案,通过精心护理,观察临床护理效果.结果：7例联合瓣、皮片均成活,1例合并糖尿病患者皮片成活,残留远端点状小创面经换药完全愈合,无肌腱或钢板外露,术后随访3～8月,联合瓣血运良好,外踝功能满意.结论：对应用腓骨肌腱鞘筋膜瓣与脂肪瓣联合转移修复外踝患者,在手术前后心理护理、术区护理、联合瓣血运监测、术后功能锻炼指导等方面的有效的护理措施,提高了联合瓣成活率,是该手术成功的重要保证.
Objective To explore nursing experiences of lateral defects were repaired by peroneal tendofascial flap conjoined with adipofasical flap.Methods From January 2008 to August 2011,8 cases of Soft tissue defects or plate exposure,were repaired by peroneal tendofascial flap conjoined with adipofasical flap.According to the patients of different ages,different conditions,for different care programs,through intensive care,observe the effect of clinical care.Results 7 cases of joint flap and skin graft survived,1 patient with diabetes,skin graft survival,remote spots a small residual wound completely healed after dressing.No tendon or plate exposure,patients were followed up 3 to 8 months,a good blood supply combined flap,lateral function were satisfactory.Conclusions On the application of peroneal tendon sheath fascia flap combined with fat transfer lateral repair in patients with psychological care before and after surgery,the surgical care,joint monitoring of flap blood flow,postoperative functional exercise guidance and other aspects of effective interventions to improve the joint valve survival rate is an important guarantee for the success of the surgery.
