The Prevention and Treatment of Cerebral Infarction Secondary to Craniocerebral Trauma

摘要 目的:对颅脑外伤继发脑梗死的患者进行治疗与预防方法的总结与探讨,寻求最佳治疗方案与预防方案,提高此类患者生存率和生存质量。方法:随机选取2012年1月至2012年6月在我院确诊并收治入院的100例诊断为颅脑外伤继发脑梗死的患者,按照随机分组的方法将患者分为实验组和对照组。其中实验组50例,对照组50例。实验组患者选择在亚低温状态下进行手术治疗,并在疾病早期进行高压氧治疗和扩容治疗,术后采用一系列针对性的预防措施。对照组患者接受传统的减压手术方法进行治疗。术后,对两组患者手术前和手术后的颅内压、水肿体积的变化等情况以及预后进行对比和分析。结果:实验组患者术后反应良好,颅内压明显降低,水肿体积明显减小,与对照组相比具有显著的意义(P<0.05);实验组预后优于对照组;实验组和对照组患者在接受治疗和康复的过程中都未出现严重的并发症和不良反应。结论:对患有颅脑外伤继发脑梗死的患者实施亚低温状态下减压手术,以及早期进行高压氧治疗和扩容治疗,效果显著优于传统的单纯减压方法,并且可以提高患者的生存率和生活质量。 Objective: To summarize and explore the therapy and prevention for patients with cerebral infarction secondary to craniocerebral trauma and to find out the best methods, with the aim of improving the survival rate and quality of life in these patients. Methods: 100 patients were randomly chosen from those who were admitted to the hospital and definitely trauma during January 2012 to June 2012, and diagnosed with cerebral infarction secondary to craniocerebral then were randomized-into treatment and control groups, with 50 cases in each group. Patients in the treatment group received operation in the condition of mild hypother- mia as well as hyperbaric oxygen treatment and volume expansion therapy during the early stage, with a se- ries of specific precautions after operation. Patients in the control group received conventional decompression surgery. After the surgery, intracranial pressure and changes in the volume of edema as well as prognosis of patients in both groups were compared and analyzed. Results: The treatment group showed slight postoperative reaction, with significant reduction in intracranial pressure and the volume of edema, and there's significant difference and better prognosis compared with control group (P 〈0.05). Severe complications and adverse reaction didn't occur in patients from both groups during the period of therapy and recovery. Conclusion: The application of compression operation in the state of mild hypothermia as well as hyperbaric oxygen therapy and volume expansion therapy during the early stage in patients with cerebral infarction secondary to craniocerebral trauma is significantly more effective than that of traditionally simple compression, while improving patients' survival rate and quality of life.
