免疫分型在单核细胞相关性急性髓细胞性白血病(M4/M5)鉴别诊断中的应用 被引量:5

摘要 目的探讨CD14、CD11b、CD64、CD4在鉴别诊断单核细胞相关性急性髓细胞性白血病(AML)(M4/M5)中的应用评价。方法采用流式细胞术分析420例急性髓细胞性白血病患者骨髓或外周血标本的免疫表型。结果在单核细胞相关性AML(M4/M5)中CD14、CD11b、CD64和CD4的表达率依次为45.4%、46.1%、83.0%和62.4%。与非单核细胞相关性急性髓细胞性白血病相比表达率高,差异有统计学意义(P<0.01)。其中CD14、CD11b特异性高(100%和91.8%),CD64敏感性高(83.0%)。结论 CD14、CD11b、CD64、CD4有助于M4/M5与其他AML各亚型的鉴别诊断。 Objective To study the differentially diagnostic value of CD14,CD11b,CD64and CD4in monocyte lineage relative acute myeloid leukemia(M4/M5).Methods Peripheral blood and bone marrow of 420cases with acute myeloid leukemia was analyzed by flow cytometry.Results In monocyte lineage relative acute myeloid leukemia,the expression percentages of CD14,CD11b,CD64and CD4were 45.4%,46.1%,83.0% and 62.4%,which were significantly higher than patients with non-monocyte lineage relative acute myeloid leukemia(P0.01).CD14and CD11b were with high specificity(100% and 91.8%)and CD64 was with high sensitivity(83.0%).Conclusion Detection of CD14,CD11b,CD64and CD4could be helpful for the differential diagnosis between M4/M5 and other types of acute myeloid leukemia.
