单纯后路撑开复位治疗胸腰段爆裂骨折疗效分析

摘要 目的 探讨单纯后路撑开复位治疗胸腰段爆裂骨折的临床效果.方法 2012年2月至2013年2月共15例胸腰椎爆裂型骨折患者,术前物理及肌电图检查无脊髓及马尾神经损伤表现.采用单纯后路撑开复位内固定术,利用后结构单纯后路撑开,恢复椎体高度,不进行椎管间接减压,避免再次损伤马尾神经及神经根.结果 15例患者均随访3~36个月,平均18个月,无血管和神经并发症,未发生断钉、螺钉松动和骨折不愈合现象.结论 15例术后复查椎体高度恢复理想,后凸畸形Cobb角明显减小,骨折块均复位且恢复或近于恢复椎管的容积,神经功能恢复较佳,表明后路撑开复位内固定术治疗胸腰段椎体爆裂型骨折是切实可行的. Objective To discuss the clinical effect of simple posterior distraction and reduction for thoracolumbar burst fracture. Methods A total of 15 patients with thoracolumbar burst fracture enrolled in hospital from February 2012 to February 2013 were without expression of spinal cord and cauda equina injury by preoperative physical examination and electromyography (EMG). Simple posterior distraction reduction and internal fixation was adopted and structure simple posterior distraction was used to restore the vertebral height and avoid injuring cauda equina and nerve root again without decompressing spinal canal indirectly. Results 15 patients were followed up for 3-36 months with average of 18 months,no vascular and neurological complications, broken nails, screws loosening and nonunion occurred. Conclusion After postoperative reexamination of the 15 patients, the vertebral height restore satisfactorily, Cobb angle of kyphosis decrease significantly, all patients are fraeture reduction with vertebral canal volume recovered or mostly recovered, and neural functional recovery is better, which show that the posterior distraction reduction fixation for thoracolumbar burst fracture is feasible.
