摘要
目的:探讨痰瘀蕴结型股骨头坏死与PPARγ信号转导途径的相关性,明确其发病机制,为防治痰瘀蕴结型股骨头坏死提供相关参考。方法:收集在广州中医药大学第一附属医院行全髋关节置换的痰瘀蕴结型股骨头坏死病例23例(实验组),髋骨关节炎病例21例(对照组),实时荧光定量PCR法检测PPARγmRNA在两组病例骨组织、肌肉组织中的表达情况,数据应用SPSS13.0统计软件分析。结果:实验组中PPARγmRNA在骨组织中的表达是其在肌肉组织中的6.710倍(t=3.4517,P=0.0260<0.05),有统计学差异;对照组中PPARγmRNA在骨组织中的表达是其在肌肉组织中的20.408倍(t=4.4745,P=0.0110<0.05),有统计学差异;实验组中PPARγmRNA在骨组织中的表达是对照组在骨组织中的1.65倍(t=3.5832,P=0.0231<0.05),有统计学差异;实验组中PPARγmRNA在肌肉组织中的表达是对照组在肌肉组织中的5.102倍(t=1.0447,P=0.3552>0.05),无统计学差异。结论:痰瘀蕴结型股骨头坏死与PPARγ信号转导途径明确相关,酒精可能会上调PPARγmRNA在骨组织中的表达,PPARγmRNA的高表达导致脂代谢异常的发生,最终导致股骨头的不可逆坏死。
Objective:To explore the correlation between the phlegm accumulation type femoral head necrosis and PPARγ sig- nal transduction pathway, and to clarify its pathogenesis, for finding better prevention and treatment of the phlegm accumulation type femoral head necrosis. Methods:The phlegm accumulation type femoral head necrosis( alcohol induced femoral head necrosis) of total hip arthroplasty in 23 patients (experimental group)were collected, while 21 cases of hip arthritis case in the First Affilia- ted Hospital of Guangzhou University of Chinese Medicine were as the control group. Real - time PCR detected the PPAR~ mRNA expression in two groups' bone tissue and muscle tissue. The obtained data was analyzed with the statistical software SPSSI3.0. Results :The experimental group PPARγ mRNA expression in bone tissue was 6. 710 times of that in muscle tissue, which had a significant difference (t = 3.4517, P = 0.0260 〈 0.05 ). The control group expression of PPARγ mRNA in the bone tissue was 20. 408 times of that in muscle tissue,which had a significant difference ( t = 4. 4745, P = 0.0110 〈 0.05 ). The experimental group PPARγ mRNA expression in bone tissue was 1.65 times than that in the bone tissue in the control group, which had a significant difference ( t = 3. 5832, P = 0. 0231 〈 0.05 ), There was no difference of the PPAR~ mRNA expression levels between experimental group and control group( t = 1. 0447, P = 0. 3552 〉 0.05 ). Conclusion : Phlegm accumulation of femoral head necrosis and the PPAR5, signal transduction pathway have a clear relationship. The alcohol may increase the expression of PPARγ mRNA, which will lead to the occurrence of abnormal lipid metabolism, eventually femoral head necrosis.
