摘要
恶性淋巴瘤是一组高度异质性疾病 ,有关其分类研究过去一直是仁者见仁、智者见智 ,数种分类 (Rappaport,L ukes- Collins,WF及 Kiel等 )也各持所长、各抒己见 .近年证实 ,各型淋巴瘤均是独立的疾病实体 ,其准确定义除了依赖病理形态与免疫表型外 ,尚需考虑其遗传特性与临床表现 .在此基础上 ,“淋巴组织肿瘤欧美修改分类 (REAL )”与“淋巴造血组织肿瘤世界卫生组织分类 (WHO)”也应运而生 .作者就淋巴瘤分类研究的历史背景、REAL 与 WHO的特点及其发展趋势作一简要回顾和复习 .
Malignant lymphomas are a heterogeneous group of disorders. Their classification has been controversial, and it has been difficult to establish an internationally accepted scheme. The controversies led to several 'competing' classification schemes, such as Rappaport, Lukes Collins, WF and original/updated Kiel and so on. Recently,it has been confirmed that each lymphoma is a distinctive disease entity, defined by a constellation of laboratory and clinical features, ie, morphologic features, immunophenotype, genetic features and clinical manifestations. Based on these features, the revised European American classification of lymphoid neoplasms (REAL) and proposed World Health Organization (WHO) classification of neoplastic diseases of lymphoid tissues were developed. In this paper the authors give a brief introduction to and a review of historic background of the classification of lymphomas and the characteristic features and developing tendency of REAL and WHO classifications.
