摘要
目的:探讨加强护理对老年慢性肺源性心脏病合并心力衰竭患者心肺功能和生活质量的影响。方法:76例慢性肺心病合并心力衰竭住院患者,随机分为常规护理组(28例,给予常规治疗、护理)与加强护理组(48例,在常规治疗基础上,加强针对性护理,指导肺功能锻炼),比较两组患者治疗前后的心肺功能和生活质量变化。结果:两组治疗后心肺功能及生活质量均较治疗前有显著改善(P<0.05或<0.01);与常规护理组比较,加强护理组治疗后生活质量评分[(49.93±8.43)分比(39.30±5.01)分]显著减少(改善);左室射血分数[LVEF(53.42±5.45)%比(61.33±5.97)%]、第一秒用力呼气量[FEV1,(53.4±5.2)%比(58.2±4.7)%]、6min步行距离[6MWD,(196±10.6)m比(203.4±9.1)m]显著增加(P<0.05)。结论:加强针对性护理可增强老年慢性肺源性心脏病合并心力衰竭患者的心肺功能,改善生活质量。
Objective: To explore effect of intensive nursing on cardiopulmonary function and quality of life in aged patients with chronic pulmonary heart disease (PHD) complicated heart failure (HF). Methods: A total of 76 inpa-tients with chronic PHD complicated HF were randomly divided into routine nursing group (n = 28, received routine treatment and nursing) and intensive nursing group (n = 48, received intensive nursing and guiding for pulmonary functional exercise based on routine treatment). Changes of cardiopulmonary function and quality of life before and after treatment were compared between two groups. Results: Compared with before treatment, there were signifi-cant improvements in cardiopulmonary function and quality of life in both groups after treatment, P〈0.05 or 0.01 ; Compared with routine nursing group after treatment, there were significant decrease in scores of quality of life [ (49.93 ± 8. 43) scores vs. (39.30 ± 5.01) scores] (indicated that quality of life was improved) and significant increase in left ventricular ejection fraction [LVEF, (53.42 ± 5.45)% vs. (61.33 ± 5.97)%], forced expiratory volume in one second EFEV1, (53.4 ± 5.2)% vs. (58.2 ± 4.7)%] and 6min walking distance [6MWD, (196 ± 10. 6) m vs. (203.4± 9.1) m] in nursing intervention group, P〈0.05. Conclusion, Intensive nursing can improve cardiopulmonary function and quality of life in aged patients with chronic pulmonary heart disease complicated heart failure.
