摘要
目的:研究长节段、短节段固定对腰骶节段退变发生过程中的影响。方法:取20具绵羊L1~S5节段标本,以椎弓根螺钉固定,依据固定长度的不同分为完整对照组、短节段固定组、长节段固定组。本实验为自身对照实验,对各组标本分别进行6Nm力矩的加载,比较各组腰骶节段活动度及椎间盘压力的变化。结果:与对照组相比,短节段固定组腰骶节段活动度、椎间盘压力增加不明显,组间差异无统计学意义(P〉0.05),长节段固定组腰骶节段活动度、椎间盘压力增加不明显,组间差异有统计学意义(P〈0.05)。结论:长节段腰椎内固定对腰骶节段退变的影响较大,建议尽量减少固定长度。
Objective: To study the changes of inferior adjacent segmental after different segmental fixa- tion, and to analyze the accelerating mechanism of the adjacent segment degeneration. Methods:Take 60 sheep lumbar spine specimens (Waist 1 vertebral body S5), With pedicle screws, according to the different fixed sec- tion is divided into complete control group (not fixed), L6-7 single segment fixed group, L5 - L7 short segmen- tal fixed group, L3 - L7 long segmental fixed group, measured biomechanical experiment in the 6Nm torque loading, L7 -S1 sacral vertebra clearance were measured in different fixed section. Results:Applied in fixed, be- neath the adjacent segment in the stretch direction of angular displacement, intervertebral disc pressure is in- creased significantly, increase amplitude with fixed range of increasese. The difference between groups was sta- tistically significant(P 〈 0.05). Conclusions: After fixing spine in pedicle screws, the longer the segmentation is fixed, the more effect it has on subjacent segment degeneration.
