亚临床甲状腺功能减退症对新诊断的2型糖尿病糖代谢的影响 被引量：4

摘要 新诊断的2型糖尿病病人307人进行甲状腺功能测定。将上述人群中合并SCH者作为观察组(n=30例),对照组(n=50例),比较两组间血糖水平及胰岛功能指标的变化。结果SCH观察组空腹血糖(FBG),餐后2小时血糖(2hPBG)明显低于EU对照组(P<0.05);促甲状腺激素(TSH)、胰岛素分泌功能指数(HOMA-β)明显高于EU对照组(分别为P<0.001和P<0.05);胰岛素抵抗指数(HOMA-IR)两组间无统计学差异(P>0.05);胰岛素C肽释放实验显示SCH观察组0min(cp0)、30min(cp1)、60min(cp2)C肽明显高于EU对照组(P值分别为0.034,0.047和0.049);相关分析表明FBG、PBG与TSH呈负相关(r值分别为-0.260和-0.327,P值分别为0.020和0.003),C肽释放试验0min、30min、60min、120min(cp3)C肽及HOMA-β与TSH呈正相关(r值分别为0.034、0.330、0.269、0.231和0.292,P值分别为0.007、0.003、0.016、0.042和0.008)。结论在年龄、性别和体质指数(body mass index,BMI)匹配的情况下,合并轻度SCH使新诊断2型糖尿病患者血糖水平降低,c肽水平、HOMA-β升高,HOMA-IR无明显改变,且随TSH升高血糖降低、胰岛功能改善更明显。
