摘要
目的探索基层医院合理使用抗菌药物管理中存在的问题,制定合理使用抗菌药物的管理对策,以提高临床合理使用抗菌药物。方法对2009年1月-2012年5月医院门诊及住院患者抗菌药物使用率进行调查,通过临床实践总结分析合理使用抗菌药物工作中存在的问题及解决的对策。结果采取动态科学指导与行政干预相结合的综合措施,2009年1月-2012年5月门诊处方抗菌药物使用率从44.20%下降至16.8%;住院患者抗菌药物使用率从77.34%下降至65.60%;Ⅰ类切口抗菌药物使用率从100.00%下降至45.01%;抗菌药物使用强度从68.48%下降至65.69%。结论单一的行政干预或单一的科学指导均很难促使临床医师做到合理应用抗菌药物,只有采取多方位的动态科学指导与行政干预相结合的综合措施才能有效提高抗菌药物的临床合理应用。
OBJECTIYE To study the existing problems in the management of rational use of antimicrobial agents in primary hospital so as to make the management strategies for the rational use of antimicrohial agents. METItODS The Jan 2009 to May 2012, the utilization rates of antibiotics of the outpatients and hospitalized patients were investigated. The problems and countermeasures of the rational use of antimicrobia] agents were summarized and analyzed through clinical practice. RESULTS The comprehensive measures of administrative intervention combined with dynamic science guidance was taken. From Jan 2009 to May 2012, the prescription rate of antibiotics in the outpatient department decreased from 44. 20% to 16. 8~/~; the utilization rate of antibiotics of the hospitalized patients decreased from 77. 34% to 65. 60% ; the utilization rate of antibiotics for the type I incision decreased from 100. 00% to 45. 01%; the intensity of use of antimicrobial agents dropped from 68. 48% to 65. 69%. CONCLUSION Single administrative intervention or single scientific guidance is very difficult to urge the clinician to use antimicrobial drugs rationally, only when the comprehensive measure of dynamic scientific guidance combined with administrative intervention is taken can the rational use of antibiotics he imnrov^c]
