摘要
目的:分析2例具有特异性抗-D自身抗体的自身免疫性溶血性贫血(AIHA)患者,分别输注完全相合和部分相合的红细胞,比较其输注效果,为此类患者输血提供依据。方法:采用红细胞抗体鉴定实验、直接抗人球蛋白试验(DAT)、微柱凝胶卡式方法等血清学实验,为2例AIHA患者筛选相合的红细胞输注,观察其输血前后血红蛋白水平,网织红细胞比例,胆红素及游离血红蛋白水平的变化。结果:特异性抗-D自身抗体的AIHA患者输入完全相合的RhD(-)和不完全相合的RhD(+)的红细胞后,均无输血反应。结论:对于含有抗-D特异性自身抗体的患者,必要时可输注凝集强度较弱RhD(+)的血液。
Objective：To compare the effects of transfusion for two AIHA patients who posses specific anti-D autoantibody and were transfused with red blood cells with perfect match or imperfect match and provide useful information for further study. Method：Erythrocyte antibody determinacy test, direct antiglobulin test （DAT）, microcolumn gel test were used to screen matched red blood cells for transfusion for two AIHA patients. The level of he- moglobin,reticulocyte ratio,bilirubin and dissociated hemoglobin were measured before and after transfusion. Result： Our data showed that there was no transfusion reaction in AIHA patients posses specific anti-D autoantibody when transfused with perfect or imperfect matched red blood cells. Conclusion：AIHA patients with specific anti-D autoantibody could be transfused the RhD positive blood with weaker agglutination intensity when it is necessary.
