摘要
目的:比较温州市医疗机构2006年和2011年乙型病毒性肝炎病例报告现状以及报告质量情况,为更好地规范医疗机构乙型肝炎报告制度提供科学依据。方法:依据前一年不同县(市、区)乙肝报告发病率高、中、低3个层次,2006年选取3个县(市、区)18家医疗单位,2011年选取5个县(市、区)23家医疗单位,按国家诊断标准对报告的乙肝病例和实验室检验结果进行核实。结果:2006年和2011年分别抽取1 017例和1 679例乙肝病例进行核实,乙肝病例诊断符合率分别为58.80%和63.19%;2006年乙肝诊断符合率以县(市)级医院最高,乡镇卫生院最低;而2011年以乡镇卫生院最高,省级医院最低。乙肝病原携带者判定为乙肝病例仍是造成诊断不符合的主要因素。结论:2011年较2006年乙肝诊断符合率有上升,各医疗机构需继续加大培训力度,开展乙肝病例诊断报告自查工作,同时完善院内沟通机制,不断提高乙肝等传染病的报告质量。
Objective To compare the status quo of the reports of hepatitis B cases and the quality of the reports of hepatitis B ~etvceen 2006 av, d 20 ll, and to provide scientific information for further regulating the report system of hep- atitis B in medical institutions. Methods Respectively eighteen and twenty--three medical institutions were hierarchical- ly sampled between 2006 and 2011. The experimental results of viral hepatitis B eases were verified according to the na- tional diagnosis standard. Results Totally 1 017 and 1 679 cases were verified with the experimental results in 2006 and 2011 respectively. The diagnosis coincidence rate was 58.80% and 63.19 %. In 2006, the coincidence rate of hepatitis B in prefectural hospitals and civic hospitals was the highest,and that in township health centers was the lowest. But in 2011, the rate in township health centers was the highest, and that in provincial hospitals was the lowest. The most im- portant reason for the disaccord was that hepatitis B carriers were falsely diagnosed. The rates were 81.38G and 70. 23~ respectively and had statistical significance （X2 = 219. 454, P^0.01）. Conclusions The diagnosis coincidence rate of hepatitis B increased,especially in township hospitals. In medical institution training should be strengthened and self --examination work of the diagnosis and report of viral hepatitis B should be carried out. Improving the communication mechanism contributes to the increase in the quality of the reports of infectious diseases.
