摘要
目的对后路腰椎间盘镜下椎间盘摘除术与传统手术治疗腰椎间盘突出症的疗效进行对比分析。方法将180例腰椎间盘突出症患者随机分为观察组与对照组，每组90例，对照组采用传统手术治疗，观察组采用后路腰椎间盘镜下椎间盘摘除术治疗，测定患者治疗前后血浆β-内啡肽和P物质、血清白细胞介素-6（IL-6）水平。结果观察组总有效率为98．9％，对照组为93．3％，两组比较差异有统计学意义（P〈0．05）。两组在治疗后均可增加β-内啡肽，观察组多于对照组（P〈0．05）；两组在治疗后均可减少P物质，与对照组比较，观察组减少更明显（P〈0．05）。两组均降低血清IL-6的表达，观察组降低更明显（P〈0．01）。结论后路腰椎间盘镜下椎间盘摘除术比传统手术更能减轻腰椎间盘突出症患者的疼痛，与减轻炎症反应有重要作用。
Objective To compare and analyze posterior lumbar disc endoscopic discectomy and traditional surgery in treatment of patients with lumbar disc herniation. Methods One hundred and eighty patients with lumbar disc herniation were randomly divided into two groups, the control group （n = 90 cases） and the observation group （ n = 90 cases）. The patients in the control group were operated through traditional surgery, while the patients in the observation group were operated through posterior lumbar disc endoscopic discectomy. β-endorphin, substance P and interleukin-6 （IL-6） were detected before and after treatment. Results The total efficiency was 98.9% in the observation group and 93.3% in the control group. There was significant difference between the two groups （ P 〈 0.05 ）. The levels of β-endorphin were increased after operation in both groups and it was higher in the observation group （P 〈 0. 05 ）. The levels of substabce P decreased after operation in both groups and it was lower in the observation group （P 〈 0. 05 ）. The levels of IL-6 were decreased after operation in both groups and it was lower in the observation group （P 〈 0. 05 ）. Conclusions Posterior lumbar disc endoscopic discecto-my is better than traditional surgery in alleviating the pain of patients with lumbar disc herniation, which had an important role in relieving inflammation.
