微小RNA-223在脓毒症患儿血浆中表达的临床意义 被引量:9

Expression of plasma microRNA-223 in pediatric sepsis patients and its clinical significance

摘要 目的 观察脓毒症患儿血浆中微小RNA-223(miR-223)的表达变化,探讨其与脓毒症的关系.方法 选取35例脓毒症患儿为研究对象(脓毒症组),其中22例为严重脓毒症.同时选取20例术后全身炎症反应综合征(SIRS)患儿(SIRS组)和15例健康儿童(健康对照组)作对照,通过实时荧光定量PCR (RT-PCR)法检测血浆中miR-223的表达水平.受试者工作特征(ROC)曲线评价血浆miR-223、降钙素原(PCT)和CRP等指标对脓毒症的预测价值.结果 各组儿童年龄、性别之间无明显差异.脓毒症组和SIRS组血浆miR-223的表达水平均较健康对照组显著升高(P均<0.05),脓毒症组血浆miR-223表达水平较SIRS组显著升高(P<0.05).严重脓毒症患儿血浆miR-223的表达水平较一般脓毒症患儿显著升高,差异有统计学意义(P<0.05).miR-223和PCT、CRP预测脓毒症的ROC曲线下面积分别为0.919(95% CI:0.851 ~0.988),0.796(95% CI:0.679 ~0.912),0.531 (95% CI:0.363~0.698).结论 脓毒症患儿血浆中miR-223表达水平升高,可以作为脓毒症的早期诊断和判断炎症严重程度的标志物之一. Objective To investigate the changes of plasma microRNA-223 (miR-223) in pediatric sepsis patients and its relationship with sepsis.Methods There were 35 children with sepsis enrolled in the study (sepsis group),in which 22 patients had severe sepsis.Meanwhile,20 patients with post operative systemic inflammatory response syndrome(SIRS) (SIRS group) and 15 healthy children(normal control group) were selected as control group.The expression levels of plasma miR-223 were detected by real-time quantitative PCR(RT-PCR).The predictive value of miR-223 in plasma,procalcitonin (PCT) and C-reactive protein (CRP) to sepsis were evaluated by receiver operating characteristic (ROC) curve.Results There were no significant differences in the age and gender among each group.The plasma miR-223 expression levels in sepsis group and SIRS group were upregulated compared with those in the norrmal control group (all P < 0.05),the plasma miR-223 expression levels in sepsis group significantly increased than those in SIRS group (P < 0.05),the plasma miR-223 expression levels in severe sepsis patients were higher than those of the general sepsis patients,and the difference was significant (P < 0.05).The area under ROC curve of miR-223,PCT and CRP to predict sepsis were 0.919 (95% CI:0.851-0.988),0.796 (95% CI:0.679-0.912) and 0.531 (95% CI:0.363-0.698).Conclusions The expression levels of plasma miR-223 in children with sepsis are significantly upregulated,which can be used as early prognostic diagnosis markers to reflect the severity of inflammation in some degree.
