摘要
目的 建立冠状动脉旁路移植手术中的动脉桥血管动物模型,为研究冠脉搭桥手术中桥血管的血流动力学和生理功能建立实验平台.方法 家兔10只,12个月龄左右,麻醉后分别获取右侧的股动脉远端及同侧的颈动脉各一段,二者长度相似,约1~1.5 cm.操作时注意避免损伤血管内膜.适当处理后将股动脉血管段端端吻合至颈动脉缺损处,建立桥血管模型,而股动脉缺损血管由颈动脉替代以防止下肢缺血坏死.闭合切口,动物饲养3周后获取移植于颈部的股动脉和左侧的颈动脉,分别用于鉴定血管内皮功能、血管收缩相似性实验以及经HE染色后进行病理形态学观测.结果 所有家兔无死亡.移植的股动脉血管全部保持通畅,血管外径增粗,管腔扩张,与原颈动脉接近.镜下观察提示移植的股动脉血管壁增厚,平滑肌层细胞适应性肥大,弹性纤维明显增多、增粗.绘制桥血管静息张力-苯肾上腺素浓度收缩曲线,提示桥血管壁产生的静息张力与苯肾上腺素浓度成正相关,并且与颈动脉具有相似的血管收缩性.结论 我们建立的桥血管动物模型,可行性和重复性都比较好,对血管内皮损伤小,符合临床实际中桥血管级别的匹配以及血液流体力学要求.桥血管与原血管具有相似的解剖结构和生理功能,是一种理想的动物模型.
Objective To establish the arterial bridge conduits model for the research of hemodynamics and physiological functionduring CABG.Methods Ten rabbits,about 12 months old,were harvested for the distal right carotid arteries and the homolateral carotid artery with the same length about 1-1.5 cm after anesthetized.Great care should be taken to avoid the injury of the tunica intima during operation.The vessels were applicably processed,and then the femoral arteries were sutured end-to-end with carotid arteries to establish the artery bridge conduits model.The defect in the femoral artery was replaced by the segment of carotid arteries harvested just now in order to prevent the lower limb necrosis.After the closure of the incision,all of the rabbits were feed for 3 weeks before reoperation to take the left carotid arteries and the femoral arteries to identify the function of vascular endothelium and the similarity of vessel constriction and pathology examination after the HE staining.Results There were no deaths in all of the rabbits.The outer diameter of the bridge conduits increased and intemal diameter expanded just as the original carotid arteries without obstruction.The vessel wall became thicken because of smooth muscle adaptive hypertrophy and increase of elastic fibers.The constriction curve of resting tension of the bridge conduits behaved positive correlation under different concentration of the phenylephrine and had the similar constrictive characteristic.Conclusion The bridge conduits model we established is easy to be set up with good repeatability and feasibility and harmless to vascular endothelium.The bridge conduits are in line with the demand of bridge blood vessel level and hemodynaimics in clinical practice and also have the same anatomic structure and physiological function as the original vessel.The model of bridge conduits is an ideal animal model,which can be used in the further clinical study of the physiological function of CABG bridge blood vessels.
