玉溪市不同感染途径HIV新近感染者CD4^(+)T淋巴细胞计数自然变化 被引量:2

Studies on the Changes of the CD4^(+)T Cell Counts in HIV Patients Recently Infected through Different Infection Routes in Yuxi Prefecture

摘要 目的探讨玉溪市不同感染途径新近感染者在无抗病毒治疗干预下的CD4+T淋巴细胞计数自然变化情况及死亡情况。方法通过回顾性队列研究获得2006-2011年HIV新近感染及CD4+T淋巴细胞计数相关数据,计算不同感染途径HIV/AIDS新近感染人群CD4+T淋巴细胞计数自然变化速率并对死亡情况进行分析。结果在51例有2次及以上CD4+T淋巴细胞计数检测结果的HIV/AIDS新近感染者中月均CD4+T淋巴细胞计数自然变化速率为:-2.26cell/μL,M=-8.07(IQR:-15.36~5.25),吸毒传播感染者、异性性传播感染者和同性性传播感染者月均CD4+T淋巴细胞计数则分别为:-17.78cell/μL,-1.16cell/μL,-4.98cell/μL,差异具有统计学意义(P=0.035)。截至2012年年底,玉溪市2006-2011年检出的HIV-1新近感染者尚未死于艾滋病及相关疾病。结论玉溪市HIV新近感染者CD4+T淋巴细胞计数自然变化总体平缓。吸毒传播感染者和男男同性传播感染者月均CD4+T淋巴细胞计数自然下降速率快,需加强对这两类感染者的定期随访,以降低其死亡率。 Objective To explore the changes of CD4+T cell counts and mortality in HIV/AIDS patients recently infected through different infection routes without antiretroviral therapy in Yuxi prefecture. Methods A ret- rospective cohort analysis was conducted to determine the changes of CD4 + T cell counts and mortality in HIV/AIDS patients recently infected through different infection routes from 2006 to 2011. Results In 51 patients who had two or more times of CD4+ T cell counts, the monthly change in CD4+ T cell counts was - 2.26 cell/μL, M = - 8.07 ( IQR : - 15.36 - 5.25 ). The monthly changes in CD4 + T cell counts differed significantly among drug use transmitted, heterosexually transmitted and homosexually transmitted patients ( -17.78 celL/μL for drug use transmitted; -1. 16 cell/μL for heterosexually transmitted; and -4.98 celL/μL for homosexually transmitted ;P =0. 035 ). By the end of 2012, none of patients with HIV-1 positive diagnosed from 2006 to 2011 died from AIDS or its associated condition. Conclusion The changes of CD4 + T cell counts in recent HIV-1 infections are small in Yuxi prefecture. Both drug use transmitted and homosexual transmitted patients show significant decreases in CD4 + T cell counts and require scheduled follow-up to reduce the mortality rates.
