摘要
目的 寻找有效和无创性诊断乳腺癌的方法。方法 对可触及的 75例乳腺肿瘤进行了99mTc -MDP显像 ,将其结果与乳腺X线摄像和B超对比分析 ,全部肿瘤经病理或细胞学证实。结果 75例乳腺肿瘤中 5 0例为乳腺癌 ,2 5例为良性肿瘤。99mTc -MDP显像诊断乳腺肿瘤假阳性 5例 ,假阴性 5例 ,其灵敏度为 90 %(45 /5 0 ) ,特异性为 80 %(2 0 /2 5 ) ,准确性为 87%(6 5 /75 ) ;6 2例乳腺肿瘤行X线摄像检查 ,灵敏度为 6 2 %(2 4/39) ,特异性为 78%(18/2 3) ,准确性为 6 8%(42 /6 2 ) ;74例乳腺肿瘤行B超检查 ,灵敏度为 76 %(38/5 0 ) ,特异性为 75 %(18/2 4) ,准确性为 76 %(5 6 /74)。乳腺X线摄像和B超的灵敏度及准确性均明显低于99mTc -MDP显像 (P <0 0 5 )。结论 99mTc -MDP乳腺肿瘤显像是一种有效的无创性诊断方法 ,它与乳腺X线摄像和B超相比 ,在诊断恶性肿瘤方面较有优势 ,且价格低廉 ,容易推广使用 。
Objectvie\ To evaluate 99m Tc-MDP scintimammography as an effective and noninvasive modality in differentiating benign from malignant breast tumors.Methods\ 99m Tc-MDP scintimammography was performed in 75 women with palpable breast masses.All pathological results were obtained by excisional biopsy or fine needle aspiration. Results\ 99m Tc-MDP scintimammography was positive in 45 of 50 breast carcinoma and 5 of 25 benign breast lesions.The sensitivity and specificity were 90% and 80%,respectively.Mammography and ultrasonography had lower sensitivity and accuracy them scintimammography. Conclusion\ 99m Tc-MDP scintimammography is a simple, accurate and noninvasive test that differentiates malignant from benign breast lesions,particularly suitable for patients suspicious as malignant tumors by mammography.
