摘要
目的观察他汀类药物治疗慢性肾脏病的临床疗效,并探讨他汀类药物非降脂作用对慢性肾脏病的防治。方法以100例慢性肾脏病患者为研究对象,将其随机分为观察组和对照组各50例,观察组在常规治疗基础之上加辛伐他汀治疗。分别监测研究对象治疗前、治疗后1个月和10个月血脂(甘油三酯、胆固醇)、肾功(血尿素氮、血肌酐、内生肌酐清除率)和24h尿蛋白定量。结果治疗1个月后,两组监测指标中24h尿蛋白定量均明显下降(P<0.05),其他指标变化不明显。治疗10月后,观察组甘油三酯、胆固醇、24h尿蛋白定量、血尿素氮、血肌酐明显低于治疗前及对照组(P<0.05),而CCr明显高于对照组及治疗前(P<0.05)。结论观察组的治疗对肾脏保护作用显著优于对照组,表明他汀类药物具有肾脏保护作用,这种保护作用为非降脂依赖性的。
Objective To observe the clinical efficacy of therapy with statins in treatment of chronic kidney disease,and to explore the non-statin lipid-lowering effect on prevention and treatment of chronic kidney disease. Methods 100 cases of patients with chronic kidney disease as the research object,were divided into the observation group and the control group,50cases in the control group,while 50 cases in the observation group.Above conventional treatment,the observation group with simvastatin treatment.The two groups were respectively observed lipids (triglyceri de,cholesterol),renal function (blood urea nitrogen,serum creatinine,endogenous creatinine clearance rate) and 24 hour urinary protein quantitative before and after one and ten months treatment,then compared them between the two groups. Results After 1 months of treatment,the index of 24 hours urinary protein between the two groups were decreased significantly (P 〈 0.05),while the other inc'lexes had no obvious change.While after 10 months of treatment,in the observation group,the index of triglyceride, cholesterol,24 hour urinary protein quantity,blood urea nitrogen,serum creatinine were significantly lower than that before treatment and the control group (P 〈 0.05),and CCr was significantly higher than that in control group and before treatment (P 〈 0.05). Conclusion The the clinical efficacy for kidney protection in observation group treatment was significantly better than the control group,and the results had shown that statins protect the function of kidney,this protective effect for non lipid-lowering dependent.
