摘要
目的 :改进颌下腺摘除的传统术式。方法 :在患者站立平视的状态下确定手术切口 ,切口设计在颌下区最隐蔽处。将颌下腺导管内插入探针或细软型塑料导管 ,深约 3~ 5cm。术中在游离颌下腺时 ,通过触摸可判断颌下腺导管的位置。凡未穿颌下腺的颌外动脉、面前静脉只结扎腺支 ,将动、静脉与腺体分离 ,予以保留。结果 :2 8例颌下腺摘除术中 ,18例颌外动脉、2 6例面前静脉予以保留。人体正常结构的完整性也得到保存。结论 :改进的颌下腺摘除术式优于传统的术式。
Objective:To improve the traditional method of extirpating submaxillary gland.Methods:Incision was determined when the patient stood and looked straightly,and the incision was in the most concealed position of submaxillary area.A probe or thin light plastic tube was inserted into the submaxillary gland duct,with a depth of 3~5 cm.The position of the submaxillary gland duct could be determined through feeling when freeing the submaxillary gland.If external maxillary artery and anterior facial vein didn't pass through the submaxillary gland,they could be separated from the gland and thus conserved after ligating gland branches.Results:Among 28 operations of extirpation of the submaxillary gland,external maxillary artery and anterior facial vein were conserved in 18 and 26 cases respectively.Thus the integration of normal structures of human body was preserved.Conclusion:The improved method of extirpating submaxillary gland was superior to traditional ones.
