先天性甲状腺功能减低症患儿生长发育与铁营养分析

摘要 目的分析先天性甲状腺功能减低症(Congenital Hypothyroidism,CH)患儿甲状腺激素替代治疗后,在6个月时体格、智力发育、缺铁性贫血的发生情况,重视及时干预,更好地促进甲低患儿生长发育。方法采取病例对照的方法,病例组为2010年9月至2012年6月,经新生儿筛查确诊的甲低患儿41例,对照组为同期到我院儿保科就诊的同龄儿童67例,均在6个月时,评估体重、身高、Gesell发育商(DQ),铁营养指标,包括RBC、HGB、MCV、MCH、MCHC,利用SPSS16.0对两组随访结果进行统计分析。结果 1.先天性甲状腺功能减低症儿童经早期治疗和干预后,没有低体重发生,有一例双胎低出生体重儿,6月龄时身高由均值-3SD提高到-2SD。2.病例组DQ值平均为98.34,对照组DQ值为97.61,两者间无统计学差异。3.病例组6个月时贫血发生率高于对照组;病例组贫血发生率31.7%,对照组为20.9%,两组比较具有统计学差异(P<0.05)。结论 1.CH患儿通过新生儿筛查早期诊断、早期治疗可避免矮身材智力低下等不良后果的发生。2.CH患儿在早期诊断、规范治疗原发病的同时,更要注意营养缺乏性疾病的定期诊治,避免因铁缺乏及其他营养素的缺乏对患儿造成的其他伤害。 Objective: To analysis the Congenital thyroid function to reduce disease (Congenital Hypothyroidism, CH) with thyroid hormone replacement therapy, in 6 months physical, intellectual development, iron deficiency anemia, pay attention to the occurrence of timely intervention, to better promote a low growth of children. Methods: Take case control method, the case group for September 2010 to June 2012, the newborn screening diagnosed with a low 41 casesand control during the same period for our son to the families of the children of the same age a 67 cases, all at 6 months, evaluation weight, height, DQ, iron nutrition indicators, including RBC, HGB, MCV, MCH, MCHC, use SPSS16. 0 to two groups of follow - up results were statistically analyzed. Results : 1. congenital thyroid function to reduce disease children by early treatment and intervention, no low weight occurs, a twins low birth weight babies, 6 months by the mean height - 3 sd increased to 2 sd. 2. the case group DQ value for an average of 98.34, control group DQ value of 97.61, no statistically significant differences between them. 3. the case group 6 months anemia rate higher than those in the control group; The case group anemia rate was 31.7%, to 20. 9% in the placebo group, two groups of comparisons with statistical difference (P 〈 0. 05). Conclusion: 1. CH children through the neonatal screening early diagnosis and early treatment can avoid short stature low intelligence, such as the occurrence of adverse consequences. 2. CH, children in the early diagnosis and treatment of the primary disease of standard at the same time, more attention should be paid to nutritional absence of sex of disease diagnosis and treatment on a regular basis, avoid the iron deficiency and other nutrients to the lack of any other children hurt.
