B超引导经皮肝穿射频治疗肝癌 被引量:22

Percutaneous ultrasound guided radiofrequency ablation therapy for liver tumors

摘要 目的 评估集束电极射频 (radiofrequency ,RF)治疗肝癌的临床疗效及适应证。 方法 复习连续的2 5 0例肝癌病人经多弹头射频治疗的临床资料 ,比较治疗前后瘤体内血供状况、肿瘤大小、AFP值变化及病人自觉症状的改善情况。结果 AFP值术后 1周内下降者占 5 0 % ,病人自觉症状明显改善。瘤体直径 <5cm的肝癌 ,其术后 1~ 6个月大部分瘤体血供消失 ,瘤体缩小 ,甚至消失。个别瘤体复发经再次射频治疗得到控制。直径>5cm的肝癌 ,配合以介入栓塞治疗 ,多数瘤体缩小。继发性肝癌 ,就瘤体而言 ,射频治疗术后大部分瘤体缩小得到控制。结论 B超引导下多弹头射频治疗肝癌近期疗效确切 ,而且满意。 Objective To evaluate the efficacy of ultrasonically guided radiofrequency (RF)thermal ablation of malignant hepatic tumors with a clustered electrodes.Methods Clinical records on 250 consecutive patients who underwent RF tissue ablation were analyzed. The vascular flow to tumor nodules, radiographic images, tumor size, clinical findings, and AFP value of before and after the procedure were compared.Results AFP value decreased in 50% of patients in a week after ablation. For tumors less than 5 cm in diameter, the vascular flow to tumor nodules disappeared, the tumors reduced in size or even diminished within 1 to 6 months after the procedure. The minority of recurrent tumors or new tumors were controlled by reablation. For tumors >5 cm in diameter, RF ablation of liver tumors was used in conjunction with transcatheter arterial chemoembolization, most tumors size reduced after the procedure.For liver metastases in so far as tumor nodule,most tumor nodules reduced in size after RF ablation.Conclusion Ultrasound guided RF thermal ablation may be an effective, safe, minimally invasive technique for treating malignant hepatic tumors.
