摘要
目的 探讨E 钙粘蛋白在人肝癌组织中的表达和癌细胞DNA含量与肝癌浸润侵袭特性之间的关系。方法 应用免疫组织化学S P法研究了 5 6例肝癌组织中E 钙粘蛋白的表达 ,同时应用ICM病理DNA图像分析系统对癌细胞DNA含量进行测定 ,分析与肝癌浸润侵袭特性之间的关系。结果 正常肝组织中 ,E 钙粘蛋白表达主要位于两细胞连接处的包膜上 ;肝脏良性病变中E 钙粘蛋白表达同正常肝相类似 ;肝癌中E 钙粘蛋白表达明显减低 ,减低率为 72 % (41/ 5 6 )。直径大于 3cm以上的肝癌E 钙粘蛋白表达减低较小于 3cm的肝癌更为显著 (P <0 0 5 )。无包膜的肝癌中E 钙粘蛋白表达与包膜完整的肝癌相比也明显降低 (P <0 0 5 )。E 钙粘蛋白表达与肝癌组织类型、组织分级等其它恶性指标未见明显相关性。肝癌组织的DNA指数 (DI)与癌灶的包膜情况、组织类型、组织分级也有显著相关性 (P <0 0 5 )。DI与其它临床病理特性未见显著相关性。结论 由E 钙粘蛋白介导的细胞间粘附作用在肝癌发生发展过程中发挥重要作用 ,其在癌组织中表达减低是肝癌重要的恶性生物学特征。结合癌细胞DNA含量测定 ,可以更准确地判断肝癌的恶性程度及生物学行为。
Objective To evaluate the relationship between expressions of E cadherin in human hepatocellular carcinomas and DNA content of cancer cells and the invasive characters of HCC. [WT5”HZ]Methods[WT5”BZ] The expressions of E cadherin were observed in 56 HCC cases using immunohistochemistry technique, and DNA content of cancer cells were analyzed by DNA imaging analyses system in order to find their relationship with invasive characters of HCC.[WT5”HZ] Results[WT5”BZ] In normal liver, immunostaining of E cadherin were observed at the lateral membranes of hepatocytes. The similar expression patterns were also found in benign tumors of liver. Analysis of HCC lesions revealed that the majority (72%, 41/56) showed markedly decreased expression of E cadherin. The expression levels of HCC larger than 3?cm in diameter significantly reduced when compared with that of less than 3?cm ( P <0 05). The HCC without encapsulation showed decreased expression levels than well encapsulated tumors ( P <0 05). There was, however, no significant correlation between these aberrant E cadherin expressions and histological type, tumor grade or other invasive characters. We found that increased DNA content of HCC correlated with non encapsulation, compact type and high grade of the tumor ( P <0 05). No relationship was observed between DI and the other clinical pathologic index.[WT5”HZ] Conclusions[WT5”BZ] Intercellular adhesion mediated by the E cadherin plays an important role in morphological changes both in benign and malignant hepatic diseases and the down regulation of E cadherin is a significant malignant character of HCC. We can give more accurate description on the malignancy and biologic behaviour of HCC with the help of DNA content assay.
