摘要
[目的]分析回肠造口术后并发症发生情况及其影响因素。[方法]回顾性分析2001年1月—2011年12月在本院行回肠造口术病人的资料,单因素分析采用卡方检验,多因素分析采用Lo-gistic回归分析。[结果]回肠造口术315例,有113例病人术后发生并发症,其中造口局部坏死5例,造口周围皮肤炎症50例,造口回缩狭窄13例,造口黏膜脱垂24例,造口旁疝21例,67.26%的并发症出现于术后1年内;单因素分析及多因素Logistic回归分析结果显示影响总并发症发生率的因素为年龄、造口治疗师(ET)指导和术后时间,影响造口周围皮肤炎症发生率的因素为ET指导、术后时间,影响造口旁疝发生率的因素为年龄、ET指导和术后时间。[结论]年龄、ET指导和术后时间是影响回肠造口术后并发症的主要原因,在ET指导下进行造口术和术后护理是降低造口并发症发生率、提高病人生存质量的重要手段。
Objective：To analyze occurrence of complications in patientsafter ileostomy and its influencing factors.Methods：Retrospective analy-sis was carried out for the data of ileostomy patients in our hospital from January 2001to December 2011,univariate analysis used the chi-squaretest,multivariate analysis used Logistic regression analysis.Results：315cases underwent ileostomy,postoperative complications occurred in 113patients,including 5cases with stoma local necrosis,50cases with in-flammation of the skin around the stoma,13cases with retracted stomastenosis,24cases with stoma mucosa prolapse,21cases with parastomalhernia,67.26% of complications occurred in 1year after operation;Uni-variate analysis and multivariate Logistic regression analysis showed thatthe influencing factors of incidence of total complications included age,ETguidance and postoperative time,the influencing factors of incidenceof inflammation of the skin around the stoma were ET guide and postop-erative time,the influencing factors of incidence of parastomal herniawere age,ET guide and postoperative period.Conclusion：Age,ET guid-ance and postoperative time are the main reasons influencing ileostomycomplications,ostomy under the guidance of the ET and postoperativecare are the important means to reduce the incidence of stoma complica-tions and improve patients＇quality of life.
