摘要
目的报告 5 0例经皮球囊二尖瓣狭窄成形术 (PBMV)的临床经验及疗效评价。方法采用Inoue法对 5 0例二尖瓣狭窄患者施行PBMV ,并对其中 38例进行术后随访。结果PBMV手术成功率 10 0 % ,无 1例死亡 ,所有患者术后血流动力学及心功能显著改善 ;二尖瓣瓣口面积 (MVN)由 0 98± 0 2 3cm2 增至 2 14± 0 2 4cm2 (P <0 0 1) ,左房平均压 (MLAP)由 3 42± 0 97kPa降至 1 5 2± 0 6 2kPa(P<0 0 1) ,左房内径 (LAD)由 5 10±± 0 87cm减至 4 5 5± 1 0 2cm(P <0 0 5 )。 38例平均随访 16个月 ,除 1例发生再狭窄外 ,其余病例手术后当时比较无显著性变化 ,LAD有进一步缩小趋势。结论PBMV近期远效果较好。严格选择病例、掌握好房间隔穿刺技术及对房颤病人常规抗凝准备是PBMV成功和减少并发症的关键。
Objective To investigate the immediate and long-term effects of percutaneous balloon mitral valouloplasty(PBMV) for the patients with mitra stenosis. Methods PBMV was performed on 50 patients with mitral stenosis, 38 cases of them were taken 16-month follow-up. Results After operation, mitral valve area increased from 0 98±0 23 cm 2 to 2.14±0.24 cm 2 ( P< 0 01),and MIAP decreased from 3.42±0.kPa to 1.52±0.62 kPa ( P< 0 01);LAD reduced from 5.10±0.87 cm to 4.55±1.02cm ( P< 0 05).Except 1 cases, the mitral of the 38 patients taken 16month follow-up has no significant change Conclusion PBMV can achieve short-term and longterm. Besides selecting the cases carefully, experimental skill and anticoagulation before operation for patients with atrial fibrillation are important factors for PBMV.
