肿瘤放疗化疗患者呼吸道感染病原菌及药敏分析

摘要 目的:分析肿瘤放化疗患者呼吸道感染病原菌的分布以及病原菌的药敏试验结果。方法:选择肿瘤患者120例,患者放疗化疗以后,取痰和咽拭子标本送检,比较分析各种病原菌,描述病原菌分布,后对病原菌进行药敏试验,探讨不同病原菌对不同抗生素的耐药情况。结果:病原菌中革兰阴性杆菌占41.8%,革兰阳性球菌22.6%,真菌33.7%。感染菌依次为白色念珠菌27.4%,肺炎克雷伯菌14.1%,大肠埃希菌11.2%,金黄色葡萄球菌9.8%和铜绿假单胞菌7.5%。革兰阴性杆菌对青霉素类和头孢菌素类耐药率高;革兰阳性球菌对红霉素和环丙沙星的耐药率较高。结论:肿瘤放疗化疗患者呼吸道感染病原菌主要分布在革兰阴性菌、真菌以及革兰阳性球菌这三大类菌种;革兰阳性菌和革兰阴性菌对青霉素类等抗生素有较高耐药性,应根据耐药菌株不同合理使用抗生素。 Objective To analyze the distribution of respiratory tract infection pathogens of patients who with tumor radiotherapy and chemotherapy, and the results of drug sensitivity of pathogenic bacteria. Method We selected 120 cases of tumor patients in our hospi- tal. After they had radiogharapy and chemotherapy, the had sputum and throat swab specimen, and analyzed varlons pathogenic bacteria,distributior of pathogens, and had drug sensitivity test to pathogens. Results the pathogenic bacteria were gram - negative bacilli accounting for 41.8% ,gram positive cocci of 22. 6%, fungi of 33.7%. Infections were Candida albieans of 27.4% ,Klebsiella pneumonia of 14. 1%, Escherichia coil of 11.2% ,Staphylococcus aureus of 9. 8% and Pseudomonas aeruginosa of 7. 5%. Gram - negative bacilli had higher re- sistant rate to penicillius and cephalosporins;gram positive cocci resistant to erythromycin and ciprofloxacin. Conclusion Tumor radiothera- py and chemotherapy patients with respiratory tract infection pathogens are mainly distributed in Gram -negative bacteria,fungi and gram - positive cocci ; Gram - positive bacteria and Gram - negative bacteria have relatively high resistance to penicillin and other antibiotics, so we should use rational antibiotics on the base of different resistant strains.
