我院近20年剖宫产率及剖宫产指征变化的临床分析 被引量:6

摘要 目的 分析北京市门头沟区医院妇产科近20年剖宫产指征的变化,探讨降低剖宫产率的有效措施.方法 选择北京市门头沟区医院妇产科1993年1月至2012年12月收治的分娩产妇16 992例,其中剖宫产8230例,从剖宫产的各项指征变化分析导致高剖宫产率的首要因素.结果 1993 ~2000年剖宫产率呈上升趋势,2000 ~2010年剖宫产率维持在56% ~63%的高水平,2011 ~2012年剖宫产率呈下降趋势.2000 ~2012年剖宫产各项指征情况显示,难产所占比例由1993 ~1999年的年均28.2%降至年均19.2%(χ2 =7.81,P<0.01),社会因素所占比例由年均8.8%上升为年均22.5%,差异有统计学意义(χ2 =9.31,P<0.01).结论 从导致剖宫产的指征入手,可逐步降低剖宫产率,降低母儿近远期并发症,确保剖宫产对母儿利大于弊. Objective To analyze the indications for Cesarean section of Department of Obstetrics and Gynecology,Mentougou District Hospital of Beijing for the last 20 years, and explore effective measures to reduce the rate of Cesarean section. Methods There were 16 992 cases of maternal childbirth analyzed retrospectively from January 1993 to December 2012 in the hospital, including 8230 cases of Cesarean section, the primary factor causing the high rate of Cesarean section through the variation of the indications was analyzed. Results Cesarean section rate from 1993 to 2000 showed a rising trend, from 2000 to 2010 remained at a high level of 56% to 63% ,from 2011 to 2012 showed a downward trend. Among various indi- cations of Cesarean section dystocia proportion fell from an annual average of 28.2% of 1993-1999 to 19.2% of 2000-2012 ( X2 = 7.81 ,P 〈 0.01 ), the proportion of the social factors rose from annual average of 8.8 % to 22.5 %, the difference was statistically significant ( X2 = 9.31, P 〈 0.01 ). Conclusion We can gradually reduce the Cesarean section rate and reduce the short and long term complications of the mother and child to ensure that the advantages outweigh the disadvantages of Cesarean section on the mother and child, starting from the indications.
