摘要
目的评估单侧颞下颌关节强直患者采取裂隙式关节成形术、关节盘复位+关节成形术方式的临床治疗效果。方法通过对本院施行了裂隙式关节成形术12例、关节盘复位+关节成形术9例的单侧颞下颌关节强直的患者进行随访观察分析,随访1～3年不等,对其开口度进行观察。结果关节盘复位+关节成形术组术后1～3年患者开口度优于裂隙式关节成形术,且好评率高于裂隙式关节成形术组。结论关节盘复位+关节成形术优于裂隙式关节成形术,在关节盘能找到并可利用的情况下,应尽可能行关节盘复位。
Objective To assess the clinical outcomes of unilateral temporomandibular joint （TMJ） ankylosis, with the surgery of gap arthroplasty or arthroplasty of repositioning of articular disc used as interpositional material. Methods Twentyone cases in the First Affiliated Hospital of Universty of South China, which were unilateral TMJ ankylosis patients treated from January 2006 to October 2011 with the surgery of gap arthroplasty （12 cases）or arthroplasty of repositioning of articular disc （9 cases） ,were followed up from 1 to 3 years to investigate the maximal mouth opening. Results The av erage maximal mouth opening and the percentage of the good effective cases of the group with repositioning of articular disc were higher than the group with gap arthroplasty from 1 to 3 years postoperatively. There was statistically significant differ ence （ P = 0.005）. Conclusion The clinical outcomes of arthroplasty of repositioning of articular disc is better than that gap arthroplasty, so we should preserve reposition of articular disc if the residual disc can be found and used as interposi tional material.
