摘要
【目的】观察自拟补益清化生肌汤合生肌散治疗子宫切口愈合不良的临床疗效。【方法】将98例剖宫产术后子宫切口愈合不良患者随机分为治疗组50例和对照组48例,治疗组给予自拟补益清化生肌汤内服,同时用生肌散蒸热后外敷腹部切口区域,连用21 d;对照组给予头孢他啶、缩宫素静脉滴注,同时口服宫血宁胶囊,10 d后改服新生化颗粒11 d。观察2组总体疗效和未愈合切口体积、恶露干净时间的变化情况。【结果】(1)治疗组的痊愈率和总有效率分别为60.0%和100.0%,对照组分别为12.5%和89.6%,2组的总有效率比较差异无统计学意义(P>0.05),但治疗组的痊愈率和总体疗效优于对照组(P<0.05)。(2)治疗后,2组的未愈合切口体积均显著缩小(P<0.05),且治疗组的缩小程度明显高于对照组(P<0.05)。(3)治疗后,治疗组恶露干净时间明显短于对照组,差异有统计学意义(P<0.05)。【结论】自拟补益清化生肌汤和生肌散联合运用能有效改善切口局部血液供应、止血消肿、抗炎和促进子宫收缩修复,并能提高切口痊愈率,值得推广运用。
Objective To observe the clinical effect of self-prescribed Buyi Qinghua Shengji decoction combined with Shengji powder in the treatment of abnormal uterine incision healing. Methods Ninety-eight patients with abnormal uterine incision healing after the cesarean section were randomly divided into treatment group （N=50）and control group（N=48）. The treatment group was given oral use of self-prescribed Buyi Qinghua Shengji decoction combined with external use of Shengfi powder on the abdominal incision area after steaming, and the treatment lasted 21 days. The control group was given intravenous drop infusion of Ceftazidime and Oxytocin, and also given Gongxuening capsules orally for 10 days. And 10 days later, the control group was given oral use of new Shenghua granula alone for 11 days. After treatment, the total therapeutic effect was evaluated, and the unhealed incision size and time for lochia disappearing were investigated. Results There was no significant difference in the total effective rate between the two groups （P〉0.05）, but the cure rate and overall therapeutic effect in the treatment group were superior to those in the control group （P〈0.05） . The unhealed incision size was obviously shrinked in both groups after treatment （P〈0.05 compared with that before treatment）, and the shrinkage was obvious in the treatment group （P〈0.05） . Time for lochia disappearing in the treatment group was shorter than that in the control group, and the difference was significant （P〈0.05）. Conclusion Self- prescribed Buyi Qinghua Shengji decoction combined with Shengji powder can improve the focal blood supply of incision, stop bleeding, reduce swelling, relieve inflammation, promote the contraction and repair of the uterus, and promote the healing of uterine incision.
