应用捕获酶联免疫测定法估算2010-2012年浙江省重点人群人类免疫缺陷病毒1型新发感染率 被引量:21

Estimation of new HIV-1 infection in high risk groups with BED-CEIA in Zhejiang,2010-2012

摘要 目的估计浙江省2010-2012年重点人群人类免疫缺陷病毒(human immunodeficiency virus,HIV)新发感染情况及其发展趋势。方法收集浙江省2010-2012年665例HIV哨点监测阳性样本和4030例新报告确证阳性病例样本,应用BED HIV-1捕获酶联免疫测定法(BED HIV-1 capture enzyme immunoassay,BED-CEIA)检测新发感染,利用公式I=[F×(365/W)×R]/[N+F×(365/W)×R/2]×100%估算人群的HIV-1新发感染率,并对同期新报告确证阳性病例计算新发感染比例。结果浙江省2010-2012年HIV监测哨点重点人群的男男性行为人群(men who have sex with men,MSM)HIV-1新发感染率最高,分别为3.52%(2.22%~4.83%)、6.63%(4.65%~7.94%)和6.33%(4.83%~7.84%),其他人群均较低。浙江省2010-2012年报告阳性病例新发感染比例分别为27.08%、26.09%和30.35%,2011年和2012年MSM报告阳性病例新发感染比例分别为40.17%和42.05%。结论浙江省MSM人群HIV-1新发感染率2010-2011年增长较快,2011-2012年稍有下降但仍维持较高水平,其他人群相对较低。报告阳性病例中MSM人群新发感染比例也较高。浙江省MSM人群HIV流行形势仍然非常严峻,应加大对该人群的干预力度。 Objective To estimate the new infection rate of HIV-1 in high risk groups in Zhejiang province from 2010 to 2012. Methods New infection was detected by using 665 positive samples collected through HIV/AIDS sentinel surveillance and 4030 samples collected from newly reported confirmed cases in Zhejiang during this period with BED HIV-1 capture enzyme immunoassay (BED-CEIA). Results The new HIV-1 infection rate in men who have sex with men (MSM) was 3.52% (2. 22% -4. 83%) in 2010, 6. 63% (4. 65% -7. 94%) in 2011 and 6. 33% (4. 83% - 7.84% ) in 2012,respectively. The proportion of positive samples in total BED tested samples was 27. 08% in 2010, 26. 09% in 2011 and 30. 35% in 2012,respectively. The new infections in MSM accounted for 40. 17% in 2011 and 42. 05% in 2012 of total MSM cases. Conclusion The incidence of HIV-1 infection in MSM increased rapidly from 2010 to 2011, slightly decreased from 2011 to 2012, indicating that the prevalence of HIV-1 infection in MSM is still high in Zhejiang. It is very important to strengthen the intervention in MSM.
