重组人组织型纤溶酶原激酶衍生物对急性心肌梗死患者BNP及临床疗效的影响 被引量:4

摘要 目的探讨重组人组织型纤溶酶原激酶衍生物对急性ST段抬高型心肌梗死(STEMI)静脉溶栓治疗患者B型脑钠肽(BNP)及临床疗效的影响。方法选择初发STEMI患者122例,随机分为两组,研究组58例给予重组人组织型纤溶酶原激酶衍生物治疗,对照组64例给予尿激酶静滴治疗,两组其余常规治疗相同。观察溶栓后血管再通率,测定血浆BNP浓度。结果溶栓后2h研究组血管再通率高于对照组(P<0.05);治疗后24h两组BNP水平均较治疗前升高,对照组升高更明显(P<0.05),1周后BNP水平回降,研究组下降更明显(P<0.05)。结论重组人组织型纤溶酶原激酶衍生物更能够有效开通梗死相关血管,抑制BNP的分泌,改善临床预后。 Objective To study the effect of recombinant human tissue type plasminogen kinase derivative on brain natriuretic peptide(BNP) and clinical effectiveness in patients with ST segment elevation myocardial infarction(STEMI).Methods 122 patients with STEMI were divided into two groups,58 patients in study group were treated by recombinant human tissue type plasminogen kinase derivative.64 patients in control group were treated by urokinase,the other of treatment were same in the two groups.Recanalization rate of infarction related artery(IRA) and BNP in the two groups were observed after treatment.Results The recanalization rate in study group was significantly higher than that in the control group after 2 hours of thrombolytic therapy(P&lt;0.05).The levels of BNP in two groups were significantly elevated after 24 hours,the control group increased more obvious(P&lt;0.05),but the level of BNP were reduced after 1 week,and study group were lower than that in the control group(P&lt;0.05).Conclusion Recombinant human tissue type plasminogen kinase derivative could open IRA,lower the level of BNP and improve the clinical prognosis.
