Phosphorus MRS study in bone and soft tissue tumors

摘要 目的 用磁共振磷谱分析的方法观察骨及软组织肿瘤的磷脂代谢和能量代谢改变。方法 在 2 .0T的场强下 ,对 14例骨及软组织肿瘤 (良性 6例 ,恶性 8例 )患者和 19例健康志愿者进行磁共振磷谱分析和常规MRI检查。测量波谱中各代谢产物的峰下面积 ,分别以三磷酸腺苷 β峰 (β ATP)、三磷酸腺苷 (ATP)、磷酸肌酸 (Pcr)为参照计算各代谢产物的相对比值 ,根据无机磷 (Pi)相对于Pcr化学位移的变化计算细胞内pH值 ,进行相对定量分析。结果 良、恶性骨及软组织肿瘤组的磷酸单脂 (PME) / β ATP、Pcr/PME、PME/ATP ,恶性肿瘤组的pH值 ,良性肿瘤组的低能磷酸盐 (LEP) /Pcr较正常组为高 (P <0 .0 1)。良、恶性肿瘤组Pi/Pcr,恶性肿瘤组磷酸双脂 (PDE) / β ATP、PDE/ATP、LEP/Pcr、Pi/ β ATP及良性肿瘤组LEP/ β ATP较对照组有显著性差异 (P <0 .0 5 )。良、恶性肿瘤组Pcr/PME、pH值的差异有显著性意义 (P <0 .0 5 )。结论 在体磁共振磷谱可以无创性地检测到骨及软组织肿瘤的磷脂代谢和能量代谢的异常变化。 Objective To study the metabolite changes in bone and soft tissue tumors using phosphorus MRS for better understanding of the phospholipid metabolite and energy metabolite of tumors, which will provide more information for clinical diagnosis and therapy. Methods Phosphorus MRS and MRI were performed in 14 bone and soft tissue tumor patients (benign 6,malignant 8) and 19 healthy volunteers at 2.0 T The areas under the peak of various metabolite in spectra were measured. The ratios of the other metabolite related to β ATP, ATP, and Pcr were calculated. Intracellular pH was calculated according to the chemical shift change of Pi relative to Pcr. Results The ratios of PME/β ATP、PME/ATP、Pcr/PME in both benign and malignant group, intracellular pH in malignant group and LEP/Pcr in benign group were higher than that of the normal group significantly ( P <0.01).The ratios of Pi/Pcr in benign and malignant group, PDE/ATP、PDE/β ATP、LEP/Pcr、Pi/β ATP in malignant group and LEP/β ATP in benign group were significantly different from that of the normal group( P <0 05). Between benign and malignant tumors group, the ratios of Pcr/PME and Intracellular pH were different significantly( P <0.05). Conclusion The in vivo phosphorus MRS can non invasively find abnormal phospholipid metabolite, energy metabolite and pH changes in bone and soft tissue tumors.
