摘要
目的探讨成冬生主任医师采用"辛开苦降"法治疗慢性胆囊炎的临床经验。方法从中医理论肝主疏泄,协助脾胃运化和主胆汁分泌的功能特点入手,分析慢性胆囊炎的基本病机及病理变化。从而寻找中医药辨证治疗该病的有效方法。结果疾病初期,肝胆气郁,湿热搏结,胆腑不通为其主要病机,清热利湿,疏肝利胆是慢性胆囊炎最常用的治疗法则。疾病继续发展,病机特点为中焦气机阻滞,寒热错杂,虚实夹杂。治当辛开苦降,寒热并用,和胃消痞,疗效显著。结论成冬生采用"辛开苦降"法,灵活运用半夏泻心汤加味治疗慢性胆囊炎或胆囊术后综合症而症见心下胃脘痞满者,临床疗效显著。
objective to study chief physician Cheng Dongshengg clinical experiences of using ＂Pungent dispersing and bitter descending method＂ to treat chronic cholecystitis. Methods from the theory of traditional Chinese medicine, liver, spleen ＇and stomach with features to assist and bile secretion, and basic disease pathology changes of chronic chole- cystitis. In order to find an effective TCM syndrome differentiation method for the treatment of this disease. Results the onset of the disease, the main pathogenesises are liver - gallbladder qi stagnation, connection of dampness and heat, ob- struction of gallbladder, clearing heat and removing dampness, soothing liver and promoting bile secretion are the most commonly used treatment principle of chronic cholecystitis. If disease continued to develop, the characteristics of patho- genesis was that the middle - jiaog Qi was blocked, cold and heat in complexity, deficiency - excess in complexity. Used pungent dispersing and bitter descending method, treating with cold and heat, harmonizing stomach and dispersing abdom- inal mass, got a curative effect. Conclusion Cheng Dongsheng used ＂Pungent dispersing and bitter descending method＂, flexible used Banxia Xiexin Decoction to treat chronic choleeystitis and gallbladder surgery syndrome and symptoms of heart of epigastric fullness, have got obvious clinical curative effect.
