精确腹腔热灌注化疗治疗中晚期结直肠癌的临床疗效观察 被引量:24

摘要 目的:探讨精确腹腔热灌注化疗(hyperthermic intraperitoneal perfusion chemotherapy,HIPC)在中晚期结直肠癌(colorectal cancer,CRC)患者治疗中的疗效及安全性。方法:采用BR-TRG-I型高精度体腔热灌注治疗系统对我院2009年12月至2012年12月收治的25例中晚期CRC患者行HIPC。每例患者治疗3次。灌注液为生理盐水(3~5L),治疗温度43℃,灌注时间90min,灌注过程中严密监测患者生命体征变化.观察患者灌注后毒副反应。灌注后1周评估患者病情缓解情况,进行KPS评分。对所有患者进行随访,评估腹水变化及肿瘤进展状况。结果:所有患者顺利完成HIPC,成功率为100%。灌注治疗期间患者生命体征无明显异常,无与治疗相关的严重并发症发生。患者的生活质量尤其是合并恶性腹水的患者得到了较大改善,治疗后2周的KPS评分较治疗前上升10—30分,平均升高(17.60±7.79)分(P〈0.01)。10例合并恶性腹水的患者9例完全缓解,1例部分缓解,有效率100%。25例患者中,失访病例4例,死亡12例,存活9例,中位生存期(18.00±9.87)个月。结论:精确HIPC作为中晚期CRC患者的辅助治疗手段安全可行,能够改善患者生活质量.对预防和治疗CRC腹膜种植转移及其并发的恶性腹水具有显著疗效。
