摘要
目的观察具有祛痰泄浊作用的加味瓜蒌薤白半夏汤法对冠心病(痰浊证)患者颈动脉硬化的改善作用。方法将63例冠心病(痰浊证)患者,随机分为治疗组31例,对照组32例,对照组予常规治疗,治疗组在常规治疗基础上加服加味瓜蒌薤白半夏汤,疗程为6个月。于治疗前后用彩色多普勒超声检测患者颈动脉内中膜厚度(IMT)、管腔内径(ΔD)和血流动力学参数[收缩期峰值血流速度(SV),舒张末期最小血流速度(DV),搏动指数(PI),阻力指数(PI)等],同时观察中医痰浊证候积分的变化。结果治疗组治疗后IMT、SV、DV、RI、ΔD较治疗前有明显改善(P<0.05或0.01),其中IMT、DV、RI、ΔD亦较对照组治疗后有显著改善。治疗组治疗后所有中医症候均较治疗前有显著改善(P<0.05或0.01),且除胸闷痛外治疗组治疗后中医证候均较对照组治疗后有明显改善。结论祛痰泄浊法对冠心病(痰浊证)患者颈动脉硬化有改善作用。
Objective: To observe the improved effect of carotid atherosclerosis syndrome by Modified Gualouxiebaibanxia Decoction in patients of coronary heart disease with phlegm syndrome.Methods: 63 cases of coronary heart disease(phlegm syndrome) patients were randomly divided into the treatment group with 31 cases and the control group with 32 cases.The control group was given the conventional treatment and the treatment group was used the Modified Gualouxiebaibanxia Decoction on the basis of conventional therapy.After treatments lasting for 6 months,some indices were observed and compared between 2 groups including the monitor carotid artery inti ma-media thickness(IMT),the lumen diameter(ΔD) and the hemodynamic parameters,which consisting of the systolic peak flow velocity(SV),the end-diastolic minimal flow velocity(DV),the pulsatility index(PI),and the resistance index(PI) etc,with color Doppler ultrasound.Meanwhile,turbidity of phlegm syndrome were measured in these groups.Results: After treatment,IMT,SV,DV,RI,ΔD in the treatment group were obviously improved than those before treatment.Also,IMT,DV,RI,ΔD were significantly improved than the control group after treatment.After treatment,all TCM symptoms were significantly improved compared with before treatment.All TCM symptoms except chest pain after treatment in treatment group were more improvements than those in the control group.Conclusion: Eliminating phlegm method has improved effect on carotid atherosclerosis syndrome in patients of coronary heart disease with phlegm syndrome.
