摘要
目的了解临床疑难配血现象,探讨应采取的下一步的措施和解决方案。方法对2010年6月至2013年2月全部配血标本中发生的疑难配血现象经进一步试验的结果数据进行分析研究。结果 A、B、O血型标本4 980例,其中正反定型不一致者22例(0.44%),其不一致的原因为抗原减弱3例(13.64%),抗体减弱4例(18.18%),A、B、O亚型3例(13.64%),近期输血3例(13.64%),冷凝集素干扰9例(40.91%);在近年14 560例临床试验检测结果中,不规则抗体阳性者26例(0.18%),其中,非特异性抗体2例(7.69%),特异性抗体24例(92.31%)。结论解决疑难配血的关键是结合患者病历资料和试验结果,分析交叉配血不合的原因,选择合适的鉴定方法做进一步试验,及时为受血者提供准确血型的合适血液。
Objective To understand clinical difficult and complicated blood matching phenomenon,and analyze the measures should be taken and solutions in the next step.Methods From 2010 June to 2013 February,the result datas of difficult and complicated blood matching in all matching blood specimens were analyzed by further experiment.Results The 4 980 cases of ABO blood specimens,including 22 cases of positive and negative stereotypes not identical（0.44%）,the inconsistent reason was due to attenuated antigen in 3 cases（13.64%）,attenuated antibody in 4 cases（18.18%）,3 cases of ABO subtype（13.64%）,3 cases of recent blood transfusion（13.64%）,9 cases of cold agglutinin interference（40.91%）;In recent years,there were 26 cases of irregular antibody positive（0.18%） in 14 560 cases of clinical trial results,including the non-specific antibody in 2 cases（7.69%）,24 cases of specific antibody（92.31%）.Conclusion The key to solve difficult blood matching is the reason analysis of incompatible cross matching,and should be combined with clinical data and results of the test,select the appropriate identification method to further experiment,promptly provide accurate blood type for blood recipients.
