摘要
目的总结应用PHILOS钢板治疗肱骨近端骨折的临床疗效。方法 37例肱骨近端骨折患者,骨折根据Neer分型:2部分骨折12例,3部分骨折18例,4部分骨折7例,均采用PHILOS钢板治疗,术后随访包括X线检查评估骨折愈合情况,并采用Neer肩关节功能评分标准评估术后1年的功能。结果 33例患者获得随访,随访时间12～36个月,平均17个月。均获得骨性愈合,平均愈合时间3.4个月。术后1年按Neer评分标准:优11例,良18例,中3例,差1例,优良率为87.9%。Neer评分最高100分,最低65分,平均86.2分。结论应用PHILOS钢板治疗肱骨近端骨折疗效满意,尤其在骨质疏松患者中更具优势。
To discuss the therapeutic effect of Open Reduction and Internal Fixation of proximal humeral fractures using PHILOS plate. Methods From Sept 2007 to Sept 2011,37 PHILOS plates were implanted in 23 women and 14 men, with a mean age of 48.1 years （range： 25 - 78 years）. According to Neer classification,there were 12 patients with two-part fracture, 18 with three---part fracture , 7 with four-part fracture. Postoperative follow-up included shoulder X-ray and functional assessment with Shoulder Neer score. Results 33 patients were followed up for 17 month（12--36 months）, Healing of the fracture occurred average in 3.4 months, avascular necrosis of 2 patient. The clinical result ac- cording to the Neer score： exeellentll, good18, fare3, bad1. Conclusion Internal fixation with the Philos plate seems to be a reliable option in the operative treatment of proximal humeral fractures, especially in osteoporotic bone.
