摘要
目的:探讨临床药师利用自身专业知识、协助临床医生对老年患者进行药学监护的方法。方法:临床药师结合患者疾病特点及药品不良反应等情况,对右颌下肿物切除术后的老年患者在抗感染药、抗高血压药与抗糖尿病药选择方面提出了药物治疗建议。结果:经过临床药师的参与,临床医生对患者进行了用药方案的调整,在抗感染药物的选择方面根据药物敏感性试验结果选药及抗菌药物不良反应的发现与应对措施,在基础疾病降压与降糖药物的选择提供了参考意见。结论:临床药师通过结合患者的实际情况,积极开展药学监护,可协同临床优化给药方案。
OBJECTIVE：To explore the approaches for clinical pharmacists to use their professional knowledge to help clinician in providing pharmaceutical care for elderly patients. METHODS： Based on the characteristics of diseases and adverse drug reactions, clinical pharmacists came up with suggestions on choices of anti-infective drugs, antihypertensive drugs and antidiabetic drugs for elder patients after excision of right submandibular mass. RESULTS ： With the participation of clinical pharmacists, clinician adjusted the dosage schedule for patients, and reference suggestions were provided such as the choice of anti-infective drugs should be based on drug susceptibility test, the detection of and the coping strategies for adverse drug reactions of antibiotics and the choice of antihypertensive drugs and antidiabetic drugs for underlying diseases etc. CONCLUSION： Clinical pharmacists＇ carrying out pharmaceutical care based on patients＇ practical situation contributed to optimization of dosage regimen.
