摘要
目的 研制一种对松质骨创面出血既有良好止血作用又有促进修复作用的复合制剂。方法 16只健康杂种犬 ,于 1侧髂骨板钻取 3个相邻的直径 10mm的全厚层骨缺损 ,分别用止血成骨胶、骨蜡、明胶海绵填充 ,术后 4、8、12、16周分 4批处死动物取标本。动物处死前 ,麻醉状态下开腹 ,显露L2 、L3、L4 椎体 ,在其正中钻取直径 16mm的圆形松质骨创面 ,分别用上述 3种止血剂填附创面 ,观察记录止血效果。结果 止血成骨胶组松质骨创面出血在 1min内完全停止且粘附牢固 ,组织学观察 ,止血成骨胶被完全吸收且被新生骨替代 ;骨蜡组创面出血虽 1min内停止 ,但组织学观察 ,术后 16周骨蜡仍不能被吸收 ,创面未能骨性修复 ;明胶海绵组既无良好止血作用又无创面修复作用。结论 本配方研制的可吸收止血成骨胶是一种临床价值较大的松质骨创面专用止血剂和修复填充材料 ,进一步研究可望成为骨蜡的替代产品。
Objective To prepare a self made compound, hemostatic osteosis jelly, which has the hemostatic and osteogenetic effect on injured spongy bone. Methods Three round bone defects of 10 mm in diameter were subjected on 16 health hybrid dogs by drilling through their either iliac plate, and were filled with hemastatic osteotic jelly, bone wax and Gelfoam respectively. Before specimens harvest, anterior surface of L 2 to L 4 bodies of vertebras were exposed, and round defects of 16 mm in diameter were drilled in spongy bone surface of the vertebral bodies and three kinds of hemostatic agents mentioned above were applied to each defects. Specimens were harvested at 4,8,12,16 weeks postoperatively for histological observation. Time of hemostasis were observed and recorded. Results Bleeding of injured spongy bone stopped within 1 min after the application of hemastatic osteotic jelly and bone wax, but not of Gelfoam. With histological observation 16 weeks after the operation, hemastatic osteotic jelly was found being absorbed and replaced by osteogenous tissue completely, and bone wax was not absorbed at all. No new formative bone tissue was observed at bone wax and Gelfoam filled area. Conclusion The compound hemostatic osteosis jelly manifests both hemastatic and osteogenetic effects on injured spongy bone and may substitute for bone wax in clinical application.
