摘要
目的探讨急性心肌梗死(AMI)患者血清B型钠尿肽(BNP)、肌钙蛋白I(TnI)、心肌型肌酸激酶同工酶(CK-MB)水平变化及其临床意义。方法选择2010年1月至2012年5月在辽河油田第二职工医院就诊的AMI患者165例(观察组)及健康体检者60例(对照组)作为研究对象,采用化学发光微粒子免疫分析法分析治疗前后血清中BNP、TnI、CK-MB水平,以及三者单独以及联合检测的灵敏度及特异度。结果观察组患者治疗前BNP、TnI和CK-MB水平与对照组比较,差异有统计学意义(P<0.01)。治疗第7日,观察组患者血清BNP、TnI和CK-MB水平均低于治疗前;与对照组比较,观察组血清BNP、TnI和CK-MB水平虽有明显下降,但仍显著高于对照组(P<0.01)。BNP、TnI和CK-MB分别检测诊断AMI的灵敏度差异无统计学意义(P>0.05),三项指标联合检测诊断AMI的灵敏度显著高于BNP、TnI或CK-MB单独检测(P<0.01);三项指标联合检测诊断AMI的特异度高达88.3%,比BNP、CK-MB的单独检测高,差异有统计学意义(P<0.05)。结论 BNP+CK-MB+TnI联合诊断可用于AMI患者的早期诊断,可作为AMI的常规检查。
Objective To investigate the serum brain natriuretic peptide (BNP) ,cardiac troponin I (TnI) ,MB isoenzyme of creatine kinase(CK-MB) level of acute myocardial infarction(AMI) patients and its clinical significance. Methods From January 2010 to May 2012 of the Second Workers'Hospital of Liaohe Oilfield 165 patients with AMI(observation group) and 60 healthy persons(control group) were selected as the research objects. Before and after treatment,serum BNP,TnI,CK-MB level were detected and the sensitivity and specificity were detected by chemiluminescent microparticle immunoassay. Results Before treatment,BNP,TnI and CK-MB levels of observation groupwere statistically significantly different from the control group(P 0. 01) . After treatment,BNP,TnI and CK-MB levels were lower than that of before the treatment,and the difference was statistically significant (P 0. 01 ) . The BNP,TnI and CK-MB levels were significantly decreased,but still higher than that of control group(P 0. 01) . The sensitivity of BNP,TnI and CK-MB had no statistically significant difference (P 0. 05 ) ; the sensitivity of BNP + TnI + CK-MB was higher than that of BNP,TnI or CK-MB single test(P 0. 01) ; the specificity of the combination test was 88. 3% ,higher than that of BNP,CK-MB single test with statistically significant difference (P 0. 05 ) . Conclusion BNP + CK-MB + TnI can be used for AMI early diagnosis,as a routine examination of AMI.
