摘要
目的探讨经阴道彩色多普勒超声诊断仪诊断胎盘绒毛膜癌的价值。方法回顾性分析我院在2006年12月至2011年12月的收取疑似患有胚胎绒毛癌患者86例,以手术后病理结果为金标准,计算经阴道超声诊断的敏感性、特异性、阳性预测值和阴性预测值,一致率和kappa值;同时计算并比较胎盘绒毛膜癌和非胎盘绒毛膜癌子宫动脉RI和滋养层血流RI。结果 :经阴道彩色多普勒超声诊断仪诊断滋养细胞恶性肿瘤的敏感性、特异性、阳性预测值和阴性预测值分别为88.9%、65.2%、87.5%和68.2%,一致率和Kappa值分别为82.6%和0.581,胎盘绒毛膜癌子宫动脉RI和滋养层血流RI明显低于非胎盘绒毛膜癌,差异有统计学意义(P<0.05)。结论经阴道彩色多普勒超声诊断仪诊断法具有成本低,声像图特异性高,诊断准确性和鉴别性高的特点,是诊断胎盘绒毛膜癌的首选方法。
Objeetlve To dicuss the value of Transvaginal color doppler ultrasonic diagnostic apparatus in diagnosis of placental choriocarcinoma. Methods a retrospective analysis in between December 2009 and December 2009,the col- lection with suspected of embryo villus cancer patients 86 cases,with 15athologic results as the gold standard after the operation, calculation of transvaginal ultrasound diagnostic sensitivity, specificity, positive predictive value, negative pre- dictive value,consistent rate and kappa value; At the same time calculation and compare the placental choriocarcinoma and placental chorionic carcinoma of the uterine artery blood flow RI R1 and nourishing layer. Results through trans- vaginal color doppler ultrasonic diagnostic instrument diagnostic sensitivity （88. 9%）, specificity （65. 2%）, positive predictive value （87.5 % ） ,negative predictive value （68.2% ）, consistent rate and Kappa value were 82.6% and 0. 581 respectively,placental choriocarcinoma uterine arterial RI and trophoblastic blood flow RI were significantly lower than the placental choriocarcinoma,statistically significant difference （P〈0.05）. Conclusion Transvaginal color doppler ul- trasonic diagnostic apparatus diagnosis method has low cost,sonograms of high specificity,high diagnosis accuracy and to identify sex, is the preferred method of diagnosis of placental villi carcinoma.
